In this RN Field Clinical Care Manager role, you will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. The 24/7 Telephone Triage Nurses filter and triage all calls. The On-Call nurse will only get a call to see new emergent admissions, death visits or anything that cannot be handle via telephone. They will receive their schedule and will be required to go to any of our locations to provide care for their patient. Responsibilities may include completing a RN admission, visiting a patient at an ALF, nursing home facility, home, hospital, or medical facility. The On Call nurse will work directly with the patient. The total amount of visits may vary per day.
