We offer: • Competitive salary, • Comprehensive health benefits, • Paid time off, sick leave, • Meaningful, relationship-based care work with manageable caseloads, • Collaborative team culture with experienced clinical leadership, • Conduct thorough initial assessments of client needs, goals, preferences, safety, and functional abilities, • Develop individualized, person-centered care plans; review and update at least annually or following significant changes in client status, • Evaluate housing needs and assist clients with placement decisions or in-home caregiver coordinationCare Coordination & Advocacy, • Serve as the primary liaison between clients, family members, physicians, attorneys, fiduciaries, and community service providers, • Coordinate and monitor all medical appointments, specialty referrals, procedures, and post-visit follow-up instructions, • Arrange and oversee ancillary services including legal consultations, financial management support, transportation, grocery shopping, and social engagement, • Attend and participate in care conferences on behalf of clients, • Advocate for client rights, dignity, and quality of life in all settingsDocumentation & Compliance, • Maintain accurate, timely, and detailed documentation of all services rendered using the WellSky Home Care portal, • Adhere to all applicable Oregon state regulations, HIPAA requirements, and agency policies regarding client confidentiality and privacy, • Participate in on-call rotation (approximately one week per month) to ensure clients have access to support after hours, • Perform other duties as assigned by the Director of Care ServicesQualificationsRequired Education & Licensure, • Active Oregon Registered Nurse (RN) license in good standing; OR Master of Social Work (MSW) degree with relevant Oregon licensure (LCSW or LMSW)Required Experience, • Minimum two (2) years of professional experience working with older adults, adults with disabilities, dementia, or individuals with mental health conditions, • Demonstrated experience in care coordination, case management, or a similar healthcare navigation roleRequired Skills & Abilities, • Strong written and verbal communication skills, including professional documentation and clinical note-writing, • Proficiency with Google Workspace (Docs, Sheets, Gmail) and electronic care management platforms (WellSky experience preferred), • Excellent critical thinking, problem-solving, and organizational skills, • Ability to work independently in community settings while maintaining strong team communication, • Valid Oregon driver's license, reliable personal transportation, and ability to pass a Motor Vehicle Records (MVR) checkPreferred Qualifications, • Certified Case Manager (CCM) or Aging Life Care Professional (ALCP) credential preferred, • Operate a personal vehicle for client visits and community appointments, • Sit, stand, walk, and occasionally lift up to 25 lbs in a variety of settings, • Work a flexible schedule that may include occasional evenings, and participate in on-call rotation, • Pass a criminal background check in accordance with Oregon OAR 407-007-0000 and agency policy, • Pass a pre-employment drug screening consistent with Oregon law, • Provide proof of current CPR/First Aid certification (or obtain within 60 days of hire - training provided), • Provide documentation of valid Oregon professional license or registration, • Provide proof of valid Oregon driver's license and current auto insurance Equal Opportunity Employer SeaStar Senior Services is an Equal Opportunity Employer.
