Job Description About the Role We’re seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Field Assessor in The Bronx, NY. In this role, you will perform UAS (Uniform Assessment System) evaluations for members in their homes and community settings. You’ll help determine eligibility and ensure members receive the appropriate long-term services and supports to enhance their health, safety, and independence. This field-based position is ideal for an independent, organized nurse who enjoys direct member interaction, community work, and contributing to improved care outcomes. Position Overview The Registered Nurse Field Assessor is responsible for conducting comprehensive assessments, developing care plans, and coordinating services for individuals with long-term care needs. This role plays a key part in promoting quality, cost-effective, and person-centered care in accordance with state and federal guidelines. Key Responsibilities • Conduct 2–4 in-home or community-based UAS assessments or reassessments daily within the Bronx area., • Develop and update individualized care plans that address members’ medical, functional, and social needs., • Collaborate with members, families, caregivers, and providers to coordinate comprehensive care., • Identify and connect members to appropriate specialists, providers, and community resources., • Monitor member progress and adjust care plans as needed to ensure goals are achieved., • Review clinical data and referrals to ensure quality, compliance, and cost-effective service delivery., • Educate members and caregivers on care options, disease management, and available community resources., • Serve as a liaison and advocate for members, ensuring their needs and preferences are prioritized., • Mentor new clinical hires by providing field support, guidance, and training., • Partner with leadership and quality teams to improve care processes and member outcomes., • Maintain accurate and compliant documentation in accordance with organizational and regulatory standards. Qualifications Education: • Graduate of an accredited School of Nursing required., • Bachelor’s degree in Nursing (BSN) preferred. Experience: • Minimum of 4 years of RN experience, ideally in long-term care, care management, or a Managed Care Organization (MCO)., • Prior UAS assessment experience required., • Experience working with high-acuity or complex populations preferred. Licensure/Certification: • Active New York State RN license required., • Must reside in the Bronx area for this field-based role. Skills & Competencies: • Strong clinical assessment and care coordination skills., • Excellent communication and interpersonal abilities., • Ability to work independently and manage multiple field assignments., • Knowledge of community-based resources and long-term care programs., • Leadership or precepting experience preferred. Other Requirements: • Valid Driver’s License and reliable transportation preferred (public transportation acceptable)., • Availability for field visits Monday through Friday.
