From coordinating referrals to keeping patient records accurate and up to date, your work will directly impact the quality of care our patients receive. Key Duties and Responsibilities • Serves as liaison with referral sources, medical groups, health plans, and other outside agencies such as funeral homes., • Verifies eligibility for patients and maintains active patient census., • Tracks active patients across care settings (hospital, skilled nursing, outpatient), • Assists RN Case Manager in obtaining appropriate authorizations for enrollment into program, specialty consults, home health referrals and durable medical equipment as ordered by the Physician or Nurse Practitioner., • Assists RN Case Manager with coordinating referrals., • Assists RN Case Manager with managing patient care plant (e.g., updating as needed and distributing appropriate parties)., • Scheduled in-patient SNF visits with patient family/ proxy, Care Connect care team and as needed., • Assist in data entry, compiling, and managing forms, paperwork, and electronic health records., • Assist in maintaining up-to-date documentation by the team., • Obtain additional clinical records required to facilitate comprehensive care., • Schedules and participates in Interdisciplinary Care Team meetings (ICT) and Family Conferences., • Work closely with Care Connect Billing Department to ensure patient records and documentation follows CMS requirements, • Adhere to the practice of confidentiality regarding patients, families, staff, and the organization., • Minimum High School Education, • Certified Medical Assistant or LVN preferred, • Experience in a medical setting is required, • Experience working as part of an interdisciplinary team, • Meet the expectations of our internal and external customers in providing excellent service, • Demonstrate positive customer relationship skills, • Thrives in an unstructured, start-up environment, • Self-starter that can work independently and collaboratively, prioritize tasks and has initiative and excitement to take on unfamiliar tasks., • Advanced knowledge of word processing, graphic presentation and computer software related to specific tasks, • Demonstrated excellent computer and word processing skills with special emphasis on calendaring, presentation, and spreadsheet capabilities, • Working knowledge of company policies, procedures, and operations, • Excellent composition, grammar, and business language skills, • Excellent communication and interpersonal skills with the ability to effectively communicate with all levels of management, patients, and family members., • Creative, flexible, well organized, resourceful, and detail-oriented, • Excellent judgment in handling confidential and sensitive information, • Ability to work independently, set priorities and handle multiple tasks with a high level of efficiency, • Establishing and maintaining cooperative working relationships with others, • Ability to work across locations and time zones, • Instills trust, • Customer focus, • Manages ambiguity, • Collaborates
