Job Description Intake & Growth Coordinator (San Jose) Ready2Nurse Home Health Position Summary As Intake & Growth Coordinator at Ready2Nurse, you own the front of our patient workflow and are the first voice referral partners and families hear, and the person who turns referrals into admitted patients: working our hospital portals and inbound patient leads, winning the patient’s choice, confirming home health eligibility, and gathering the records our clinical and QA team needs. You also manage growth initiatives: maintaining and building relationships with our existing referral sources and patients and ensuring a steady intake of patients in the door. Since we are a small company, you may also support other roles and assist as needed in all areas of patient care coordination. Qualifications: Required 3+ years in home health, specifically in intake. Warm, confident, and persuasive phone presence: able to win the confidence of patients and referral sources in the first call and build relationships that last. Hands-on insurance eligibility and prior authorization experience: running Medicare eligibility, checking commercial coverage through Availity or payer portals, recognizing when authorization required, and initiating and tracking auth requests. Excellent organization skills - ability to track and manage multiple patients at various stages of intake, and work fast under deadlines. Working knowledge of Medicare home health eligibility and regulations. Comfortable with light field work: valid driver’s license, reliable vehicle, and willingness to occasionally drive within the greater San Jose area. Preferred Experience with hospital referral portals (AIDA, Careport). Clinical Background or degree WellSky / Kinnser (or comparable home health EHR) experience. OASIS Expertise and knowledge of Home Health regulations Job Duties: Referral Intake You will primarily be responsible for “winning” patients who have contacted us initially – getting patients from initial phone call, fax, or email, through intake “acceptance” and start of care. Continuously monitor and respond to our various intake channels – hospital portals, fax, email & phone inquiries – and respond within 1 hour, often much faster. Screen referrals for service area, clinical fit, and clinician capacity, and verify insurance eligibility (Medicare and Commercial) using Availity or other online portals; escalate questionable cases to leadership. Follow up with hospitals and physician offices for required documentation (face-to-face notes, home health orders, etc.). Patient Experience and Growth Conduct post-discharge follow-up calls at 30, 60, and 90 days; complete surveys and identify patients who may benefit from a new episode of care and coordinate with their physician for future orders. Track all discharged patients as well as patients or families who dropped out of pipeline for potential future outreach. Manage key relationships with established referral sources (physician offices, skilled nursing facilities, case managers, etc); complete in-person visits to resolve documentation or signature delays. Start of Care Coordination Coordinate a clean hand-off of each accepted patient to the Patient Care Coordinator or Scheduler for start-of-care prep and scheduling. Support SOC prep and scheduling when needed: complete pre-charting for patients before SOC, complete and deliver admission packets, coordinate SOC schedules with patient and clinician. Compensation & Schedule Compensation: $25–30/hour, with additional earning potential through our performance incentive program Schedule: full time, Monday to Friday business hours, based in San Jose with intermittent local travel.
