POSITION PURPOSE The Community Health Field Supervisor is a leadership role responsible for overseeing, supporting, and developing field-based staff within a designated geographic region. This position ensures high-quality service delivery, operational efficiency, clinical accuracy, and adherence to APH policies, accreditation standards, and contractual guidelines. The Field Supervisor conducts audits, performs ride-alongs, manages KPIs, and builds strong relationships with providers, members, and community partners. In addition to management duties, the Field Supervisor is required to maintain half caseload, providing direct member support to stay connected to frontline operations, maintain clinical competency, and supplement state needs. This position requires strong leadership, extensive travel, and a deep understanding of community-based health services. DUTIES & RESPONSIBILITIES Team Leadership & Oversight • Provide direct supervision, coaching, and development for assigned field staff., • Assist with recruiting, onboarding, training, and competency assessments., • Conduct quarterly (or more frequent) ride-alongs with each staff member., • Complete weekly chart and workflow audits for accuracy, quality, and compliance., • Ensure adherence to organizational policies, accreditation standards (NCQA/URAC), HIPAA, and clinical protocols., • Develop performance improvement plans when needed., • Monitor KPIs, outcomes, caseload distribution, productivity, and member engagement targets., • Support refinement of workflows, processes, and operational efficiencies., • Conduct home visits, assessments, and follow-up calls according to program standards., • Engage members using evidence-based outreach and documentation practices., • Support complex cases and emergent situations as needed., • Travel throughout the entire state is required. Travel outside of the state is required per management’s request and for mandatory meetings., • Build relationships with community agencies, providers, and other partners., • Partner with Operations, Quality, and Clinical Leadership to support organizational goals., • Participate in meetings, trainings, and provider or community stakeholder engagements., • Engage in continuous professional development and required trainings., • Communicate organizational expectations clearly and consistently., • Perform other duties as assigned by leadership. SKILLS & QUALIFICATIONS • Minimum of 3 years of healthcare experience, preferably in community health, case management, or care navigation., • Minimum of 1 years supervisory or management experience., • Strong understanding of field-based care delivery and documentation standards., • Exceptional communication, coaching, and relationship-building skills., • Ability to manage a half caseload while concurrently supervising a team., • Strong strategic thinking, organizational, and problem-solving skills., • Ability to travel extensively across assigned regions., • Bilingual (English/Spanish) preferred., • Experience with Medicaid, Medicare Advantage, community-based programs strongly preferred. EDUCATION, LICENSES, & CERTIFICATIONS • Bachelor's degree preferred., • Associate’s degree strongly preferred., • Medical Assistant degree required; Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), or Certified Clinical Medical Assistant (CCMA)., • Basic Life Support (BLS) certification required.
