POSITION OVERVIEW The Care Manager for Children’s Health Home provides patients advocacy, outreach, education, and care management. Skills and competencies required for the position include communication, cultural competence, training professional experiences, and education. The Care Manager for Children’s health home works closely to collaborate services to link at-risk youth to services and programs that support that support functioning in the least restrictive community setting. The Care Manager works to provide stability for at-risk youth who are chronically ill, underserved, mentally disable, and high inpatient and emergency department utilizers. This position includes no managerial or supervisory responsibilities. Job Summary Under the supervision of the Managing Director of Congregate Care, the Program Director will partner with the Social Services team to develop policies, procedures and programs within the site as well as the larger WSFSSH Community. This position is responsible for the operations of the building, staff supervision, operation of the program within the budget, facility maintenance, and compliance. Program/Department Description: Community Access’s OMH licensed Intensive Crisis Residence (ICR) is inspired by the Soteria model and is designed as an alternative to psychiatric hospitalization for people going through an acute psychiatric crisis and who would benefit from a respite period in a stabilizing, home-like environment with 24-hour peer support and treatment services. The Soteria ICR will work with guests for up to 28 days to help them develop awareness and skills that will allow them to return to the community with an increased ability to manage their mental health concerns. The ICR will follow a Soteria model which emphasizes shared running of a home-like environment, positive expectations of recovery, validation of subjective experiences, the concept of “being with” a person, providing 1:1 support, 24 hours a day, and self-determination. Services are completely voluntary, and guests are encouraged to maintain their daily responsibilities and activities in the community during their stay including work, school, family care, recreation, and wellness appointments. The program works with guests throughout their stay on discharge planning focused on safety and wellness, and provides post-discharge follow up health resource navigation support. Position Overview: The Program Director is responsible for the overall administration and the day-to-day operation of the Soteria Intensive Crisis Respite (ICR), ensuring all contractual obligations are met and all operations are mission-aligned. This includes establishing and maintaining a culture of accountable, supportive leadership and supervision that promotes quality assurance and professionalism. Job Description About the Role The Case Aide performs various supportive activities for casework services in support of the needs of the Preventive Services programs. Duties may include “support activities” such as interviewing clients; collecting information or investigating situations to obtain information about family situations; directly assisting children and family members in various ways, i.e., providing transportation, home visits, accompanying parent to service providers, participating in and reporting observations and findings at case conferences. What You’ll Do • Work closely with the Case Planners to provide collateral services to individuals and families. Attend JHV with Case Planners and CPS at initiation of case., • To assist and/or model for our parents (caretakers) and children methods to achieve their service plan goals. To participate in supportive services such as recreation and educational programs which support behavioral goals for the children., • To carry out ongoing quarterly risk and safety assessments of the family and clearly document these assessments in Connections and in their progress notes. Bring all risk/safety concerns to the immediate attention of their supervisor., • To request all school and medical reports; document in Connections utilizing appropriate progress note template and file in case record. Place printed copies of all important documents in case files., • To assist the families in accessing in-house and community-based services to address issues that place a child at risk of foster care placement and promote maximum safety, supervision and support for children and their families.
