We are hiring community support staff to begin as soon as possible. 2901 Juan Tabo Blvd NE, Suite 208 Summary: The CCS Coach is part of the Great Livin' team responsible for helping the Individuals we support to live their greatest lives. The CCS Coach is a member of an Interdisciplinary Team (IDT) responsible for the implementation of the Individual Support Plan (ISP) of the person served, as well as providing for the health and safety of those Individuals on a daily basis. The CCS Coach is most directly involved with the individuals and is responsible for assisting them in reaching goals related to leisure activities, recreation, life skills, and community involvement. The job description of the CCS Coach can be categorized into the following general areas; • Provide direct support and assistance to the Individuals for which you are responsible, • Complete required documentation, • Complete required administrative responsibilities, • Create and support linkage to community Roles and Responsibilities PROVIDE DIRECT SUPPORT AND ASSISTANCE TO THE INDIVIDUALS FOR WHICH YOU ARE RESPONSIBLE Support and assist the Individual with completing their daily needs through: • Obtaining the Individual Specific Training needed to understand the Individuals needs and proper means for providing support and assistance, • Ensuring the Individuals tend to their personal needs as applicable and as independently as possible, including, but not limited to: attending community activities, participating in online activities with individuals, supporting the individual to build community relationships., • Ensure transportation to activities, classes and appointments, • Adapting the environment to help meet the needs of the Individual, • Work with Individuals' personalized ancillary therapy programs (OT, PT, SLP, BSC) and supports, • Provide behavior supports according to positive behavior support strategies and individualized needs as outlined in the ISP and/or PBSP, • Assist and support the Individuals to develop strategies utilized to make informed decisions and develop a meaningful day, • Support and assist the Individual with ISP Outcomes and Action Steps related to CCS, • Utilize Teaching and Support Strategies as outlined in the Individuals ISP, • Provide the opportunities and support to the Individuals to engage in completing their identified ISP Outcomes Support and assist the Individuals with ensuring their health and safety needs through: • Implementing the applicable plans including, but not limited to: Health Care Plans; Comprehensive Aspiration Risk Management plans; Meal Time Plans, • Assist with medication COMPLETE REQUIRED DOCUMENTATION • Ensure that all required, applicable documentation has been completed before leaving your shift, including, but not limited to ISP Data Notes, MAR's, GER's, Health Care Tracking, Vehicle and Facility Checklist. COMPLETE REQUIRED ADMINISTRATIVE RESPONSIBILITIES • Ensure that you have clocked in/out properly using the timekeeping system, • Attend all mandatory meetings, agency, and State trainings, • Maintain confidential information LINKAGE TO COMMUNITY • You are a representative to the community for the Individuals we support, • Finding/creating opportunities for those receiving services by Great Livin' to be a part of their community, including, but not limited to, such things as becoming connected with an area church or synagogue, joining the neighborhood association, participating in volunteer activities, joining the Lobo or Isotopes booster club, volunteer organizations etc., • Encourages and supports the maintenance and development of natural supports, social roles, and vocational pursuits/interests of the Individual's in the home Additional Tasks • Other tasks as assigned by the Great Livin' LLC Directors, management, and supervisors relevant and within the guidelines mission and ethics of Great Livin' LLC, • Completion of job-related requirements per Great Livin's Policies and Procedures, and State Standards, Procedures and expectations (including – as applicable, but not limited to: completion of all required documentation before leaving shift; completion of State Abuse, Neglect and Exploitation Reporting; GERs; Time IPS) Knowledge and skills: • Must be able to speak, read and write in English, • Proficient in the use of a personal computer and various software packages, • Must develop a working knowledge of Therap, timekeeping system, and other agency systems as applicable, • Knowledge of agency positions and responsibilities, and who to go to with specific questions and/or concerns, • Must be of good integrity and possess adequate physical, mental, and emotional stability so as to meet job duties in a safe and responsible manner, • Must have good organizational and time management skills, • Must develop a general understanding of the DD and Mi Via Waivers, and a working knowledge of Standards for which you are directly responsible (e.g. Supported Living, Customized Community Supports), • Able to communicate in a professional, knowledgeable and well-organized manner with co-workers, Individuals, and Team members (e.g., Case Manager, Guardian, Family Member, BSC, SLP), • Able to establish priorities, work independently, and proceed with objectives without hands-on supervision, • Able to handle and resolve recurring problems Experience / Educational Requirements: • Must possess a high school diploma or GED, • Must be at least 21-years old, • Agreeable to additional training/s when appropriate for the enhancement of position skills Expected Job Outcomes • Monitoring and support of the Individuals with whom you are working, • Timely and accurate filing of necessary paperwork/reports, • Household activities and tasks are operating effectively, • Concise and accurate completion of assigned tasks, • Conduct himself/herself as a member of the Great Livin' team, • Assist with the growth, quality, and direction of Great Livin', • Cohesive communication between the Great Livin' Management team, co-workers, Individuals, and relevant team members related to Great Livin' and the people we support Special Requirements • Valid New Mexico Driver's License, • Valid Auto Liability Insurance, • Reliable transportation, • Pass required background checks, • Job-Related Certification(s)/Trainings, as and when deemed necessary by Great Livin' LLC and/or the state of New Mexico
