Job Description Physical Therapist - PRN Schedule: PRN | 8:00 AM - 4:30 PM| About Nexus Neurorecovery Center – Conroe Situated on a peaceful, wooded 26-acre campus, Nexus Neurorecovery Center – Conroe offers the Rise Program, a comprehensive neurological rehabilitation program for adults recovering from severe traumatic brain injury (TBI), stroke, spinal cord injury, and other neurological diagnoses. This campus is also home to the Life Care Program, which provides long-term residential care for individuals who have sustained serious, life-altering injuries and require ongoing clinical and behavioral support. Position Summary The Physical Therapist (PT) plays a critical role in helping patients regain strength, mobility, and functional independence following neurological injury or complex medical illness. Through comprehensive evaluations and individualized treatment planning, the Physical Therapist provides hands-on, evidence-based therapy designed to improve movement, balance, coordination, and overall physical function. Working as part of an interdisciplinary rehabilitation team, the Physical Therapist treats patients who may have cognitive, behavioral, or medically complex conditions in a structured therapeutic environment. This role is ideal for clinicians who enjoy working with neurologic populations and who value teamwork, consistency, and making meaningful progress with every patient. Key Responsibilities • Conduct physical therapy evaluations and develop individualized treatment plans, • Provide therapy focused on mobility, strength, balance, coordination, and functional independence, • Treat patients with neurologic, behavioral, or medically complex conditions, • Use adaptive equipment, assistive devices, and therapeutic modalities as needed, • Monitor patient progress and modify treatment plans accordingly, • Supervise PTAs and assist with coordination of therapy services, • Collaborate with occupational therapy, speech therapy, psychology, nursing, and medical staff, • Maintain accurate documentation in the electronic medical record, • Follow Nexus policies, HIPAA standards, and safety procedures, • Use approved de-escalation techniques when needed to maintain safety, • Support a structured, therapeutic environment that promotes patient progress Licensure & Certifications • Current Physical Therapist license in the State of Texas required, • Graduate of an accredited Physical Therapy program required, • Doctor of Physical Therapy (DPT) preferred, • BLS / CPR Certification required, • De-escalation training required within 30 days of hire, • Minimum 2 years of clinical experience preferred, • Experience working with neurologic, rehabilitation, or medically complex patients preferred, • Hospital, inpatient, or rehabilitation experience strongly preferred, • Ability to work effectively within a multidisciplinary clinical team, • Compassionate, patient-centered approach to care Our Core Values: GOPTIC At Nexus, GOPTIC is more than a slogan—it’s the foundation of how we care for children and families. • Genuine – We act with honesty, integrity, and empathy in all interactions., • Ownership – We take responsibility for our actions and celebrate our shared success., • Positivity – We create a hopeful and encouraging environment for every child and family., • Teamwork – We work together across disciplines to achieve the best outcomes., • Innovation – We constantly seek better, more creative ways to support healing and growth., • Have you worked with patients who have neurological or medically complex conditions?, • Are you comfortable working in a rehabilitation setting?, • Are you available to work PRN?, • Do you enjoy working as part of an interdisciplinary therapy team?
