This role is ideal for RNs who thrive in autonomous environments and enjoy community-based patient care. As a Home Health RN, you will assess patient needs, create individualized care plans, deliver in-home clinical care, coordinate referrals, and collaborate closely with physicians and interdisciplinary team members. 🎯 Required Qualifications • Valid Oklahoma RN License (or multistate equivalent), • Graduate of an accredited School of Nursing, • Valid driver’s license, clean MVR, and personal vehicle with minimum liability coverage, • Reliable transportation for home visits, • Proficient computer skills, • Strong clinical judgment, communication, and interpersonal skills, • Home Health experience required🩺 Key Responsibilities, • Conduct patient assessments to determine eligibility and care needs, • Develop & update patient-centered plans of care, • Provide culturally sensitive, holistic nursing care in the home setting, • Review medications for safety, effectiveness, interactions, and compliance, • Document care accurately in EMR (Meditech), • Perform skilled nursing procedures per physician orders, • Educate patients & families on disease management, safety, and emergency plans, • Coordinate multidisciplinary referrals and discharge planning, • Conduct Home Health Aide supervisory visits every 14 days, • Maintain productivity and meet documentation standards, • Participate in QA, case conferencing, and performance improvement initiatives, • Weekly pay, • Medical benefits
