Key Responsibilities * Deliver comprehensive, patient-focused primary care services within a value-based care model. * Manage a patient panel of approximately 13–16 patients per day. * Conduct visits averaging 20 minutes for regular appointments and 40 minutes for new or complex cases. * Collaborate with a dedicated care team including a Medical Assistant, Scribe, and Social Worker. * Utilize on-site lab services with phlebotomy support and access to RubiconMD for virtual specialty consultations. * Maintain accurate documentation using Canopy and Greenway EMR systems. * Participate in community health initiatives such as wellness classes, exercise groups, and social engagement activities.
