Job Description Bilingual Patient Care Coordinator (English/Mandarin) – Direct Hire Location: Phoenix, AZ 85027 Job Type: Direct Hire (Full-Time) Pay Range: $22 - $24/ Hour + $2 Mandarin Bilingual Premium Schedule: Monday-Friday 8am-5pm (1 hour lunch) Position Overview Pride Health is seeking a Bilingual Patient Care Coordinator (English/Mandarin) to join a growing primary care team in Deer Valley, AZ. This is an excellent opportunity for a compassionate, customer-focused professional who is passionate about improving patient outcomes through care coordination, education, and exceptional service. The ideal candidate will have healthcare or patient care experience, Electronic Medical Records (EMR) knowledge, and the ability to communicate fluently in both English and Mandarin. This position is 100% onsite and may require occasional travel between nearby clinic locations. Key Responsibilities • Serve as the primary point of contact for patients, providing exceptional in-person and virtual support., • Coordinate specialist referrals, diagnostic appointments, and follow-up care., • Educate patients regarding referrals, treatment plans, and healthcare resources., • Support patients with chronic conditions through education, reminders, and care coordination., • Schedule appointments and manage patient service requests., • Collaborate closely with providers and the care team to ensure seamless patient care., • Assist with patient outreach programs, quality initiatives, and closing gaps in care., • Maintain accurate documentation in the Electronic Medical Record (EMR) system., • Build and maintain relationships with specialty providers and referral partners., • Ensure patient confidentiality while complying with HIPAA and organizational policies., • Perform additional duties as assigned. Required Qualifications • Fluent in English and Mandarin (spoken and written)., • Experience using Electronic Medical Records (EMR)., • Direct patient care or healthcare experience., • Strong customer service and patient communication skills., • Current CPR Certification., • Active Arizona Fingerprint Clearance Card., • Excellent organizational, interpersonal, and problem-solving skills., • Ability to work collaboratively in a fast-paced healthcare environment., • Ability to maintain strict confidentiality. Preferred Qualifications • Medical Assistant (MA) Certification., • Certified Nursing Assistant (CNA) experience., • Clinical background in an outpatient or primary care setting., • Experience with patient referrals and care coordination., • Interest in continuing education or obtaining additional certifications.
