Job DescriptionDescription: Job Summary Social Work & Behavioral Health Manager oversees the Social Work/Case management team at PACE. Ensures face to face assessment for participants along with case management services, advocacy, care planning and implementation of care plan interventions. Coaches social work team and other SW team Serves as a liaison between IDT and the social work services and behavioral health services provided at PACE. Coordinates behavioral health services including counseling, crisis intervention, group work and community engagement. Participates in orientation, clinical training, and supervision of MSW staff. Serves as consultant/advisor to PACE staff with respect to social work and behavioral health concerns. Duties/Responsibilities • Collaborates with all departments and facilitates good relationships and communication among staff., • Provides leadership and clinical guidance to the social work and behavioral health staff., • Participates in the orientation, preparation and supervision of staff LICSW licensure and/or professional development (as applicable)., • Oversees the development of plans of care for new and enrolled participants and works with social work team to ensure quality of care and timely completion of all required assessments., • Oversees the behavioral health program at PACE. Develops community resources in conjunction with the Contracts Manager. Assesses the Behavioral Health program and makes recommendations/offers solutions to modify the program to best meet participant needs., • Utilizes policies, procedures and standardized tools as guidelines when making social work/behavioral health decisions, • Participates in enrollment process to assist in behavioral health screening and assessment of needed services for potential participants., • Engages in problem solving and critical thinking on a SW supervisory level. Assists social workers, care partner liaison and other members of social work team to problem solve and manage challenging cases., • Serves on the Clinical Management Team to ensure quality clinical services and care plan integrity for participants of PACE., • Participates in quality improvement activities and/or other committees or community involvement that is consistent with the PACE mission to improve services provided., • Acts as a clinical resource for PACE staff regarding social work, ethical, behavioral health, social policy and advocacy concerns and identifies teaching opportunities related to same., • Works with IDT to develop and implement protocols to respond to chronic illness and prevent health crisis/hospitalization. Is a member of the UR team., • Conducts qualitative and quantitative medical record audits to ensure proper delivery and allocation of services, • Prepares written reports and presents information relative to care management activities to supervisor and/or key stakeholders as required or requested., • Is knowledgeable and adheres to all regulatory statutes regarding social work standards of care, state and federal regulations regarding PACE centers., • Required to provide administrative on-call coverage on a rotating basis in order to assist in the delivery 24-hour care to enrolled participants, • Protects privacy and maintains confidentiality of sensitive participant, employee, and agency information, • Works with leadership to identify systems improvement opportunities and identifies potential solutions to ensure efficient operations and adherence with PACE mission and regulations, • Serves as a role model for students and staff members., • Completes yearly performance reviews and competencies for social work and behavioral health staff., • Addresses legal and ethical concerns, acts as a participant advocate and ensures proper reporting to the authorities. Leads the Ethics committee., • Identifies areas or participants at high risk and reports to supervisor and IDT., • Participates in strategic planning goals for the Organization and leads social work team in working toward achievement of these objectives., • Create and implement departmental audit system to identify areas of non-compliance and opportunities for quality improvement; use data to inform decision making and action steps., • Works in the community at partner facilities or at participant homes or other venues, as necessary., • Demonstrates an understanding of the importance of clear, thorough documentation as crucial to good participant care; completes and enters accurate, thorough participant documentation in the electronic health record; enters documentation for assessments, orders, and notes within required timeframes., • Performs other related duties as required and assigned. Leadership & Communication • Meets regularly one-on-one with staff to provide performance-based feedback and support professional growth., • Conducts regular department meetings to communicate goals, deliverables, progress, and Senior Leadership updates., • Attends and actively participates in management meetings; shares relevant information with other managers to support alignment and collaboration., • Ensures clear, timely communication of organizational updates and departmental expectations., • Demonstrates a proactive, growth-oriented mindset focused on continuous improvement over task completion., • Maintains professionalism and fosters an approachable environment where staff feel comfortable seeking guidance., • Models and promotes company culture by encouraging engagement in organizational initiatives, events, and groups. Staff Management & Development • Delegates tasks and projects appropriately, fostering accountability and employee growth., • Supports employee development by helping staff achieve goals identified in their evaluations., • Provides role-specific orientation for new hires and arranges regular training in identified areas., • Holds staff accountable for performance and conduct; collaborates with supervisors for consistency and fairness; promptly addresses issues with HR support when needed., • Completes staff evaluations and competency assessments as required; submits thorough documentation to HR on time. Operational Oversight • Oversees timely, accurate completion of employee timecards., • Monitors and manages staff PTO to ensure compliance with rollover deadlines; manages overtime within budget limits., • Prepares and maintains departmental reports with key metrics, improvement areas, and learning opportunities based on data analysis., • Identifies quality improvement (QI) activities, risk areas, and compliance concerns; contributes to projects and ensures alignment with departmental reporting., • Maintains current and relevant departmental policies and SOPs in Policy Manager., • Ensures assigned grievances are resolved thoroughly and on time; delegates as appropriate and completes required documentation promptly. Organizational Engagement & Representation • Actively supports recruitment efforts to ensure the selection of qualified candidates aligned with organizational needs., • Promotes staff participation in educational initiatives; ensures compliance with required trainings., • Supports the organization’s internship program by hosting interns, promoting placements, and/or engaging in other supportive activities with students within areas of accountability., • Represents the organization to external stakeholders as needed and participates in organizational initiatives & events. Requirements: Required Skills & Abilities: • Ability to maintain confidentiality, • Ability to prioritize tasks and meet project deadlines, • Proficiency with MS Office Suite or similar software, • Dependable and punctual with ability to maintain consistent attendance, • Ability to read, write and comprehend English, • Ability to maintain sound judgement under stress and communicate effectively, • Ability to research and analyze data, draw conclusions, and resolve issues, • Ability to read, interpret, and apply policies, procedures, laws, and regulations Education Requirements • Master of Social Work Degree, Required Physical Requirements • Must be able to tolerate prolonged periods sitting at a desk and working on a computer., • Must be able to lift up to 10 pounds and at times up to 20 pounds., • Must be able to navigate various departments of the organization's physical premises as well as related community care settings., • Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases. Experience • 5 Years of Direct Service Work, Required, • 2 Years of Experience in a Medical or Geriatric Setting, Preferred, • 1 to 3 years Staff Management Experience, Required License & Certification Requirements • Rhode Island Licensed Clinical Social Worker (LICSW, preferred) Position Requirements • Master of Social Work Degree, • Driver's License & access to reliable transportation: community-based travel required.
