Job Description LPN Care Manager Monday–Friday 8am–5pm | Full-time | Tulsa, OK | $23–$29/hour DOE | Temp to Perm Summary: Provide patient care management for individuals requiring neurological and transitional care. Monitor patient progress, perform assessments and injections, provide education, and coordinate clinic resources. Collaborate with the primary care team to ensure patients follow care plans and meet health goals. Duties and Responsibilities: • Assist in managing patients requiring neurological and transitional care, • Monitor patient progress, vitals, and medication adherence, • Perform patient assessments, injections, and neurological exams as needed, • Conduct face-to-face and remote patient encounters, • Educate patients and families on disease management, medications, and healthy lifestyle choices, • Coordinate clinic resources to support patient care, • Communicate patient information to providers and care teams, • Schedule new and established patients and track outgoing referrals, • Maintain accurate patient records in electronic health systems, • LPN diploma from an accredited program, • Current Oklahoma LPN licensure, • Minimum one year of healthcare experience, • Knowledge of nursing care principles and patient management, • Ability to perform assessments, injections, and vitals, • Proficiency with computers and electronic health records, • Strong verbal and written communication skills, • Professional, courteous, and patient-focused, • Clinical environment with potential exposure to contagious disease, • Interaction with emotionally distressed patients and families, • Ability to lift up to 20 lbs, sit/stand, and move throughout the clinic #MED TRINITY EMPLOYMENT SPECIALISTS IS AN EQUAL OPPORTUNITY EMPLOYER See the great things people are saying by checking out our Google reviews, along with our Facebook, LinkedIn, Instagram, X/Twitter.Please visit theCareer Centeron our website for some helpful resources to help in your job search, to build a resume, for interview tips and many job opportunities! • Care for ill, injured, or convalescing patients or persons with disabilities in hospitals, nursing homes, clinics, private homes, group homes, and similar institutions. Licensing required., • Observe patients, charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, and taking any necessary action., • Measure and record patients' vital signs, such as height, weight, temperature, blood pressure, pulse, or respiration., • Administer prescribed medications or start intravenous fluids, noting times and amounts on patients' charts., • Provide basic patient care or treatments, such as taking temperatures or blood pressures, dressing wounds, treating bedsores, giving enemas or douches, rubbing with alcohol, massaging, or performing catheterizations., • Answer patients' calls and determine how to assist them.
