Role and Responsibilities The RN Care Services Supervisor is responsible for managing client care as efficiently and cost-effectively as possible, to achieve positive patient outcomes regardless of payer. This role is also highly focused on Aide retention operates within the Licensed Home Care Service Agency (LHCSA). The RN Care Services Supervisor is a dedicated to participating in our Home Health Aide Training Program (HHATP) teaching new staff to be Aides. They will follow Aides from the classroom to the field so they can transition seamlessly and apply learning to hands on patient care. This role will be responsible for developing the overall plan of care for clients and the outcome of that care, including care delivered by other staff (LPN, HHA, PCA) who must be directed and supervised by the LHCSA RN. Essential Functions RN Case Management Functions: Overall responsibilities include: providing and coordinating quality patient care while maintaining a case load/productivity as assigned by direct supervisor. Accurate and timely documentation supporting care provided and/or needs of the client. • Conduct admission assessments and determine if a safe plan of care can be administered including the need for home health aide/personal care assistance including time of day, frequency of visits and length of time required to complete the plan of care., • Case manage primarily aide-related services according to regulatory guidelines., • Conduct in-home supervisory visits to oversee care given by home health aides and personal care aides., • Practice according to best practice standards and regulatory nursing standards., • Maintain relationships and clear lines of communication with MLTC case managers and other members of the care team., • Communicate effectively with aides, provide mentoring & positive feedback through phone conversations, and visits, • Oversee clinical skills of assigned aides, provide training., • Communicate with aide preceptor regarding skills, possible support needed in field., • Document according to standards of nursing practice., • Mentor new hires with consistent oversight, • Provide leadership, supervision, and operational guidance to Home Health Aides and Personal Care Aides, including reviewing Aide productivity, performance management, employee relations, and disciplinary/counseling., • Assist in completing annual performance evaluations, • Investigate and address Aide concerns, complaints, accidents, and other issues involving peers, patients, and/or care delivery. Report to the Regional Director, Executive Director and/or Human Resources, as appropriate., • Assist clinical educator in executing actions for Aide’s development of knowledge and skills, to ensure proficiency and patient care quality., • Responsible for ensuring all assigned staff complete all in-service credits annually., • Assist in coordination of HCR Preceptor program, • Demonstrate competencies as they relate to information/technology., • Establish and maintain relationships with community partners., • Other duties as assigned. Responsibilities: The RN Clinical Coordinator II position is responsible for clinical management on the unit in coordination with Nursing Staff and management. The RN Clinical Coordinator II will be responsible for performing ongoing rounds and RN clinical assessment on high-risk patients. This position creates and reviews clinical documentation for the unit to support an effective clinical management support. • Review residents’ reports with Charge Nurse, discuss and note residents' conditions, taking note of critical Residents, physician’s orders, floor coverage, and similar details., • Supervise rounds done on the floor. Help and provide guidance where needed., • Follow up re: need for bowel/med administration for residents on an individual basis. If concerns noted, follow-up with charge nurse., • Review treatment sheets for those scheduled for assigned shift. Treatments as scheduled-confirm documentation is consistently completed., • Follow-up as needed with residents re: significant changes prior to or on prior shift. Notify DON/ADON of concerns. Contact MDs when needed for change in resident’s clinical status or a need for orders based on resident complaints, lab results, etc., • Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs., • Ensure that nursing documentation is complete, accurate and completed within the appropriate time frames., • Participate in the development and implementation of policies, procedures and standards of resident care., • Monitors unit staff compliance with all facility and nursing department policies and procedures., • Plan for, coordinate, and facilitate resident admissions and transfers to respective nursing units., • Attend and participate in family conferences., • Coordinate referrals for consulting services., • Collaborate with physician and Nurse Practitioner regarding resident problems, daily medication needs and monthly renewals., • Facilitate communication among physicians, residents, families, and interdisciplinary team members., • Function as facility supervisor as scheduled., • Participate in Nurse Manager meetings as scheduled by the Director of Nursing., • Serve as a resident and family advocate and update families on change in status of residents., • Maintain the confidentiality of all resident care information and ensure that unit staffs respect all residents’ rights., • Respond to emergency situations within the facility according to Code and Disaster policies and procedures., • Participate as a member of nursing and/or facility committees., • Assist in preparing written and/or oral reports of the nursing service programs and activities to submit to such committee(s) as directed. Qualifications: • Registered Nurse License with current registration in New York State, • Graduate of an accredited School of Nursing, BSN preferred, • At minimum, 3 to 5 years-related experience working in a health-care facility., • At minimum, 2 years of supervisory experience., • Strong verbal, written, computer and leadership skills required.
