SUMMARY: The Patient Advocate Scheduler (PAS) works closely with their assigned Home Color Team, which includes the provider, support staff and the patients associated with that team. The PAS is primarily responsible for managing all scheduling functions – this includes new appointments, cancellations, and reschedules, for their assigned color team. Additionally, the PAS provides direct support to patients, addressing any scheduling related needs and ensuring a smooth coordination of care. QUALIFICATIONS & ATTRIBUTES: • High school diploma or GED., • Must be certified (CMA, RMA, MA, EMT, CNA or similar), • Post high school education is advantageous., • Proven experience working in a medical office., • Working knowledge of medical terminology and medical insurance plans., • Proficient in Microsoft Office applications (Word, Excel, Outlook)., • The ability to type and file accurately., • Strong communication, interpersonal and presentation skills, • Good computer and electronic record skills, • Excellent organizational skills, • Excellent interpersonal skills, • Aptitude in problem-solving, critical thinking, and decision-making, • Outstanding time management skills, • Excellent written and verbal communication skills., • Uphold and actively promote the organization's mission and values in all tasks, • and interactions., • Respond to Microsoft Teams messages, emails, and voicemails daily. Ensure all, • communications are addressed and resolved before the end of your shift., • Stay current with healthcare policies to provide the best service to patients., • Maintain and routinely update assigned census; escalate discrepancies when, • necessary., • Utilize Microsoft Excel to track, analyze, and report trends related to scheduling, • and patient census data., • Assist patients/caregivers in meeting scheduling needs, including appointment, • confirmations, reschedules, and cancellations, • Verify insurance and eligibility when required to support scheduling or, • coordination of tasks., • Follow up on no-show or missed appointments to ensure continuity of care and, • timely rescheduling., • Proactively identify scheduling gaps and work to fill open slots to optimize, • provider schedules, • Collaborate with clinical and administrative staff to resolve scheduling conflicts, • or special requests., • Enroll eligible patients into Chronic Care Management (CCM) program and, • ensure proper documentation of consent., • Accurately document time spent with patients in CCM tracking software in, • accordance with program requirements., • Achieve and maintain weekly productivity recommendations., • Participate in team meetings, training sessions, and ongoing professional, • development activities., • Perform other duties and special projects as designated by the manager.
