Case Manager will be responsible for responding to housing crisis and participants concerns, supporting participants to maintain their housing, assist them with their housing transition plan, and meet their person-centered service plan goals. Case Managers are also responsible to completed efficient housing searches and support landlord & neighbor relationship building. It’s essential to collaborate with one’s care team to meet their community objectives and goals. Job DescriptionJob Title: Behavior Health Technician Reports To: Director of Program Operations FLSA Status: Non-Exempt Compensation: $18-$20/HR Saturday and Sunday Overnights 8:00pm - 8:00am Summary: The primary purpose of this role is to provide supervision for our clients and to ensure their safety while building a rapport with them. This includes providing assistance to each client in working with a group and in handling individual problems; responsibility for setting of limits for behavior; creating documentation of the clients progress; identifying the possible need for professional services and communications of such findings to professional staff. • This role requires working with adult male sex offenders in a residential setting Duties and Responsibilities • Responsible for whereabouts of residents and what they are doing at all times., • Monitor resident’s behaviours., • Facilitate residential daily schedule including participating in, and providing support to residents, to include but not limited to, activities such as chores, deep clean of designated space, meal service, laundry service, groups, activities, and events., • Carry out regular checks on residents., • To administer medication at designated times, as prescribed by the GP., • Assist with planning individual care for residents as needed., • Communicate with the Clinical staff both verbally and in writing about the residents’ general condition and developments in the home., • Rotate being on call in case emergency coverage is needed., • To answer the telephone and take messages as required., • Take residents out on outings., • Oversee visitation., • Perform some case management tasks (e.g., set up medical appointments, daily living skills, resume writing, insurance enrollment)., • Write case notes during each shift., • Practice compliance with all policies, record keeping requirements, assigned documentation, and any other assigned duties., • Ability to manage time effectively and to multi-task., • Remain current and up to date on all training and certifications required to maintain employment with Steps For Change- Adult Residential., • To have good knowledge of fire, emergency, first aid, CPR and security procedures Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
