Rapidly after release into the Supervised Release Program, the Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions required under the supervision plan. In this role, you will help participants comply with their supervision appointments and court appearances during their legal case's pendency. You will also conduct ongoing service need assessments with participants and develop collaborative service plans addressing identified needs and goals. You will ensure participants receive services that are strengths-based, gender-specific, and culturally sensitive while adhering to the core principles of risk, need and responsivity to reduce recidivism, increase engagement in community treatment services and support participants to achieve their self-identified goals for treatment, recovery, rehabilitation, and crime-free community integration. This position is based in our Brooklyn Community Office and fieldwork in the larger NYC community as needed to assist participants. CASES’ Supervised Release Program (SRP) annually diverts 7,500 people otherwise facing pretrial detention at Rikers Island to community-based supervision and support services. Program participants include youth and young adults, people with substance use disorders and mental illnesses, and individuals who are homeless. SRP participants have not been found guilty of a crime and are not mandated to engage in treatment or other services. We are dedicated to developing innovative programs that meet the complex needs of our diverse community and that align with our organizational values. Moriviv Residential Program is a 51 bed scattered site safe dwelling. The goal of this program is to provide short term, emergency shelter for people fleeing domestic violence. Program staff work with each participant to develop an individualized action plan that connects them to support services and helps them secure permanent housing. Supportive services include: counseling, support groups, case management, childrens services, referrals for educational/vocational facilities, job preparation and health promotion for women and children. The Program Manager is responsible for the overall daily operations of the program and the safe dwellings. In addition to acting as VIPs primary coordinator to assure that all efforts are fully met and exceed contractual obligations (programmatic and administrative) and direct the day-to-day activities of risk management, safety/security, maintenance and client move-in/turn-over. Job Description The Schwarzman Animal Medical Center is seeking an ABVP or ACZM board-certified or board-eligible veterinarian to join our Avian and Exotic Pet Service as a full-time clinician. The Avian and Exotic Service at AMC provides 7-day/week veterinary care for a varied caseload of approximately 2,600 animals/year comprising birds, small mammals, reptiles, amphibians, and some fish. The clinician will join a service consisting of one full-time ABVP board-certified veterinarian (Reptile and Amphibian Practice), 2 part-time ABVP board-certified veterinarians (Avian Practice), and 1 per diem clinician. The Service also includes 2 house officers (2 specialty interns or intern/resident), 4 full-time and part-time technicians, 4 full-time assistants, and 1 service coordinator. Clinical and Service Responsibilities In a 4-day work week, clinical responsibilities consist of daily clinical appointments, inpatient care, clinical procedures, and consulting on walk-in emergency cases. You will share oversight and training of house officers in avian and exotic pet medicine and surgery and in the training of general rotating interns. As a Level 1 Trauma Center, the AMC Emergency and Critical Care Department provides 24-hour emergency service. No on-call emergency duty is required. The Avian and Exotic Pet Service is fully integrated with other services at the AMC and works in close coordination with other specialists to provide optimal patient care. Specialty services include anesthesia, cardiology, dentistry, dermatology, diagnostic imaging, emergency and critical care, internal medicine, interventional radiology and endoscopy, neurology, ophthalmology, oncology, radiation oncology, rehabilitation and integrative medicine, and soft tissue, orthopedic, and oncologic surgery. Job Description AHRC NYC is seeking a Clinical Coordinator (CC) to oversee all aspects of its Crisis Respite Transition Program (CRTP) in Brooklyn.The Clinical Coordinator (CC) is responsible for collaborating with FCS and Residential staff in overseeing all aspects of operation for the Crisis Respite Transition Program (CRTP). The CRTP provides clinical and residential supports for people with I/DD experiencing psychiatric or other behavioral health crises. The CC fulfills these responsibilities through the activities of the staff he/she supervises and therefore, primary responsibilities include the training and oversight of direct care staff in appropriate delivery of qualitative person-centered, trauma informed care and treatment and consistent adherence to safety and other policies and procedures. The CC acts as a role model for DSPs in engaging with the people they support and with related stakeholders to work towards mitigating crisis-related factors and facilitating a path to a safe and stable return to the community. The CC identifies unmet needs of the people supported in the house and gaps in programming, communicates with appropriate parties, and follows up on concerns. ESSENTIAL RESPONSIBILITIES The CRTP provides trauma informed clinical and residential supports for people with I/DD experiencing psychiatric or other behavioral health crises. • Assists with the interviewing, hiring, training and oversight of direct care staff in the provision of clinically based care for the emotional, physical and personal well-being of the individuals and ensuring a nurturing, structured and protective environment, • Ensures ongoing compliance and quality assurance with State and Federal regulations applicable to operation of facility to which he/she is assigned., • Provides direct supervision and guidance to all employees assigned to the CRTP. Completes mandated written evaluations of staff in residence, as well as oversees any disciplinary actions needed to address performance problems by staff., • Ensures staff assigned to the respite house are registered for all required and appropriate training., • Provides service coverage in accordance with staff scheduling established by senior management, as well as provides ad hoc coverage in cases where scheduling may change due to direct staff shortages or emerging needs of residents., • Assumes accountability for smooth operations of the respite house, including adherence to funder’s and agency’s policies and procedures related to house and vehicle maintenance, fire safety, and residents’ and personnel files., • Collaborates with clinical and management team in planning each person’s individualized program of services and care, covering both daily and longer-term needs, and ensuring delivery of services and care by direct care staff that addresses each person’s interests, behavioral health needs, preferences, strengths and capabilities., • Participates in comprehensive service planning activities with outside agencies, as applicable, for each person at the respite site., • Monitors direct care activities related to medical services, including ongoing maintenance of electronic and paper medical records, scheduling and attending of medical appointments, appropriate oversight for individuals’ self-administration of medication with support, and request for referrals as needed., • Ensures each person’s basic needs are met, including adequate supply of food, clothing, personal hygiene items, and any other personal belongings necessary to meet individuals’ needs within the respite house., • Ensures nutritional needs of residents are fully met., • Ensures recreation and leisure activities are planned and coordinated (staffing, funds, transportation) in accordance with each person’s interests., • In conjunction with the Clinical team, reviews assessments, data collection and other relevant information, formulates and recommends new interventions directed towards enhancing the quality of each resident’s life., • Participates in clinical supervision and trainings on topics including trauma informed care, supporting people with challenging behaviors, and supporting people with severe autism., • In collaboration with all staff at the respite house, formulates and recommends new ideas and outcomes directed towards enhancing quality of each person’s life., • Attends and successfully completes mandated and skills development training courses, as assigned, as well as provides training to staff at residence, as needed., • Conducts additional duties, as needed.Qualifications REQUIRED QUALIFICATIONS • Master’s level clinician with at least 3 years of related experience required. Licensed mental health clinician preferred., • Experience in supervising staff a plus., • Must be knowledgeable in the field of developmental and intellectual disabilities and have experience working with people with both I/DD and psychiatric disorders., • Must have ability to plan, organize, develop, and implement program policies and procedures in a timely and consistent manner., • Ability to be patient, enthusiastic and confident when interacting with people supported, understanding the psychosocial and cognitive levels at which they are functioning., • Ability to deal diplomatically with personnel, people supported , family members, visitors, government agencies and the general public., • Must possess ability to make independent decisions and handle stressful situations when circumstances warrant., • Valid Tri-State New York State driver's license, in good standing and safe driving history, with no or infrequent violations/suspensions/revocations preferred.
