Job Description PURPOSE OF POSITION Covenant House Missouri (CHMO) welcomes youth who are runaway, trafficked, at-risk, or experiencing homelessness with unconditional love, absolute respect and a place to call home. Beyond shelter and basic needs, we deliver transformative solutions to help young people ages 16 to 24 build productive, independent and sustainable lives. The Case Manager is responsible for providing goal-oriented support and individualized direct care and supervision of program youth. This position is focused on achieving individualized goals for the persons served by assessment, planning, advocacy, and coordination in a manner which reflects the mission, philosophy and policies of Covenant House Missouri. SUMMARY OF DUTIES • Responsible for establishing and maintaining good relationships with appropriate boundaries with youth., • Responsible for client intake and administration of various assessments used to gain knowledge of and assist in the progression of the youth., • Assist in identifying clients' needs and create individualized treatment plans that represent the best interest of the client and ensures appropriate goals and objectives., • Initiate crisis intervention counseling to youth and to families as needed., • Collaborate with interdisciplinary team to:, • Identify the best possible approach to providing services to the youth in the Residential Programs., • Integrates and continuously supports a Trauma Informed culture thru treatment planning and daily interactions, advocating with and on behalf of individuals, according to their needs., • Provide information on and/or refer individuals to established and preferred public, private community agencies for additional services., • Responsible for developing, organizing and conducting life skills and group activities to prevent and resolve problems related to human development and self-care., • Transport and accompany youth to appointments outside of Covenant House Missouri., • Model positive behaviors and coping skills for youth, while demonstrating a strength-based approach to all interactions and utilizing aspects of Trauma Informed Care and Positive Youth Development., • Record and maintain records, required paperwork, and documentation according to agency policy & procedures., • Lead, plan, and facilitate various team and individual meetings for the purpose of sharing information in regard to youth., • Responsible for proper administration of medication and first aid per agency policies and guidelines., • Assist in planning, scheduling, and implementation of on and off site residential activities., • Establish working relationships with fellow team members to build trust and confidence. Constructively evaluate current practices and procedures to ensure effectiveness in client advancement., • Actively engage with volunteers/partners assigned to the residential programs., • Participate in all required trainings and staff meetings, completing at least 40 hours of training annually., • Act in accordance with all local, state, and federal laws, as well as organizational protocols, policies, and procedures., • Perform other duties as assigned. KNOWLEDGE, SKILLS, & ABILITIES Education & Experience Bachelor's degree in Social Work, Human Services or related field or equivalent experience required, master's degree preferred. Applicants with an unrelated Bachelor's or Master's degree, but possessing direct relevant experience and/or competencies may be considered on case by case basis. Minimum two (2) years' experience preferred. How you'll make an impact The Pharmacy Auditor is an integral part of the Pharmacy Audit Team and is responsible for performing and supporting Prescription Drug Claim Audits (claims, rebates, operational, Medicare, pre/post-implementation, etc.) to validate that the coverage of a prescription drug claim, and the payment of such claim, comply with the contractual, regulatory, and custom plan guidelines. Additionally, certain audits validate the proper reimbursement of prescription rebates from a Pharmacy Benefits Manager (PBM) to a client per the contractual arrangement. This position is client facing and requires an individual who can effectively manage client expectations as well as the ability to manage multiple projects without compromise of accuracy or timeliness. • Project management of Prescription Drug Claim Audits from implementation through closure., • Manage and perform audits of prescription drug claim payment data to validate conformance with contractual requirements and plan specifications as assigned., • Review and evaluate Administrative Services Agreement between the employer and PBM., • Work with dedicated data analytics team to define and translate plan provisions for testing purposes, ensuring that scope aligns with the statement of work and client commitments. Review client-specific documentation, Summary Plan Description(s), and other documentation such as copay grids, plan literals, set-up grids, etc., to develop the audit test filters., • Complete the contract summary, or assist with completion of contract summaries, as assigned., • Evaluate the results of the initial claim data tests and determine valid audit samples for review and response by the PBM., • Prepare PBM communications regarding test results, response expectations, and clarifications to questions., • Work collaboratively with PBM to resolve exceptions, review supporting documentation, and make final claim error determination., • Ensure audit timeline is adhered to and deliver high level of service to clients., • Perform Prescription Drug Claim Rebate audits as assigned. (Occasional travel to the PBM is required; 10-20% travel possible.), • Prepare and present detailed audit reports which clearly articulate audit findings and recommendations to PBM and client., • Maintain audit project work papers to support audit findings., • Work collaboratively within the audit team, as well as with internal and external customers., • Participate in team meetings, conference calls, Microsoft Teams meetings, etc. remotely with PBMs and audit team as necessary., • Assist with claims data analysis and quality assurance as requested., • Interact directly with clients and respond to client requests. Most of our patient population is those diagnosed with myocardial infarction requiring cardiac catheterization, congestive heart failure, heart arrhythmias, and pulmonary hypertension. • Cardiac PCU Most patients are diagnosed with congestive heart failure, heart arrhythmias, pulmonary hypertension, and other life-threatening conditions. With this known diagnosis many may require cardiac catheterization and stent placement. As a leader in advanced therapies for heart failure, we offer care to pre- and post- heart transplant and Left Ventricular Assist Device (LVAD) patients. This unit specifically cares for readmitted LVADs and manage complex issues like pump clots, stroke, and manage medications. • Vascular Surgery PCU Our nursing staff provides pre/post-operative care to the wide variety of interventions needed for peripheral vascular disease patients. Surgical services include endoluminal and open repair of thoracic and abdominal aortic aneurysms, thoracic outlet decompression, carotid artery angioplasty/stenting, carotid endarterectomies, dialysis access (fistulas/grafts), catheter-directed thrombolysis, and various bypasses and stents for aorto-iliac and upper/lower extremity occlusive disease. • Cardiothoracic PCU Post Thoracic Surgery patients ranging from lung transplants, lung surgeries, and esophagectomy. In addition, we care for the Cardiothoracic population after they stabilize from the ICU. We will take care of Pre/Post CABG, Valve Repairs, Heart transplants, and other surgical procedures. • Cardiac Surgery PCU, • We provide post-operative care to cardiac surgery patients. Our most frequent procedures are CABG (coronary artery bypass graft), Cardiac Valve Repair/Replacement, LVAD (left ventricular device implantation), Aortic repairs, and MAZE., • Cardiac ICU Patients with MIs, cardiomyopathies and a variety of irregular rhythms, including life-threatening v-fib and v-tach, device designated floor LVAD, IMPELLA, intra-aortic ballon pump, Non post operative side, pre surgical heart intervention patients. • Cardiothoracic ICU Patients are CTICU, had surgery from their esophagus to diaphragm, open heart surgery, cabbages, valve replacements, active heart failure program (heart is failing and only thing to do for is to add devices) ventricular assist device. This allows for high-quality, standardized care for all patients regardless of surgeon specialty. Additional Information About the Role Divisions 6400 & 6500 are a combined trauma, geriatric trauma, and acute care surgery nursing division. We have a combined bed capacity of 63 patients. Our nurses specialize in the care of patients who have experienced a variety of traumatic injuries including motor vehicle accidents, gunshot wounds, stab injuries, traumatic amputations, fall injuries, burns, fractures, spinal cord injuries, and traumatic brain injuries.
