Job DescriptionJob Title: Patient Care Coordinator Job Description The Patient Care Coordinator supports a high-volume clinical practice by serving as a primary point of contact for patients, coordinating tests and referrals, managing prescriptions, and maintaining accurate electronic health records. This role enhances patient flow and improves the overall patient experience by ensuring clear communication, timely follow-up, and efficient administrative support for physicians and clinic staff. Responsibilities • Respond to inbound and outbound patient phone calls, answering questions about treatments, symptoms, prescriptions, and other clinical inquiries in a clear and compassionate manner., • Coordinate diagnostic tests requested by physicians, including imaging and laboratory work, obtain required pre-authorizations, and communicate results to patients promptly and accurately., • Process prescription requests by sending new prescriptions and refills via written, faxed, or electronic methods, ensuring accuracy and compliance with clinic protocols., • Schedule outgoing referrals to specialists or other providers and transmit necessary chart information and documentation to support continuity of care., • Request medical records from other physician offices and accurately scan and upload documents into the Electronic Health Records (EHR) system., • Complete patient-related paperwork such as workers’ compensation forms, FMLA documentation, and other administrative forms, and thoroughly document all work in the EHR system., • Manage appointment scheduling to support efficient patient flow, including coordinating with other team members to accommodate high patient volumes., • Provide patient education and support, reinforcing provider instructions and helping patients understand their care plans, follow-up steps, and available resources., • Collaborate closely with physicians, medical assistants, and other clinic staff to ensure smooth daily operations and a positive patient experience in a fast-paced environment., • Utilize strong computer skills and clinic systems to maintain accurate records, track tasks, and support data integrity across all patient interactions.Essential Skills, • Minimum of 3+ years of experience as a Medical Assistant (MA)., • Hands-on experience working with Electronic Health Records (EHR) systems, including scanning, documentation, and record management., • Proficiency in appointment scheduling and coordinating patient visits in a clinical environment., • Experience handling inbound and outbound calls with patients, including answering clinical questions and managing follow-up communication., • Demonstrated patient care and patient education experience, supporting patients through their treatment plans and follow-up needs., • Strong computer skills and ability to work efficiently with multiple clinical and administrative systems., • High school diploma or equivalent (required).Additional Skills & Qualifications, • Bachelor’s degree (strongly preferred)., • Bilingual language skills (preferred), with the ability to communicate effectively with a diverse patient population., • Experience in ophthalmology or exposure to ophthalmology clinical settings is a plus., • Strong organizational skills and attention to detail, especially when handling documentation, forms, and medical records., • Ability to thrive in an extremely fast-paced clinical environment and manage a high volume of patients each day., • Excellent interpersonal and communication skills, with a focus on delivering a positive patient experience., • Adaptability and willingness to support a growing organization with opportunities for upward mobility.Work Environment This role is based in a very fast-paced clinic environment where the team may see approximately 80–100 patients per day. Work hours fall between 6:30 a.m. and 6:00 p.m., with schedules assigned based on clinic needs; early starts or late shifts are rare and typically occur only when the clinic is extremely busy. Initial training takes place at an administrative building in the Galleria area for a full day before transitioning to the clinic. You will work as part of a collaborative team of approximately 8–24 individuals, with each physician supported by about eight clinic staff members. The position involves daily use of Electronic Health Records systems, phone systems for inbound and outbound calls, and standard office and clinical equipment. The dress code requires navy blue scrubs, with short, well-groomed nails and all hair securely tied back to maintain a professional and hygienic clinical environment. Job Type & Location This is a Contract to Hire position based out of Bellaire, TX. Pay and Benefits The pay range for this position is $20.00 - $23.00/hr.
