Job Description JOB SUMMARY The Staff Physician is responsible for delivering high-quality medical care to patients across the lifespan, including preventive services, diagnosis, treatment, and ongoing management of acute and chronic conditions. The Physician collaborates with Nurse Practitioners, Physician Assistants, nurses, and other clinical staff to ensure coordinated, culturally responsive, and evidence-based care. This position supports population health initiatives, care management strategies, and quality improvement efforts. JOB DUTIES • Provide comprehensive primary and/or specialty medical care, including preventive, acute, and chronic disease management, • Obtain and document medical histories and perform physical examinations, • Develop, implement, and monitor diagnostic and treatment plans, • Order, interpret, and evaluate laboratory and diagnostic tests, • Prescribe medications and therapeutic interventions in compliance with clinical guidelines, • Educate patients on diagnoses, treatment options, medications, and preventive health measures, • Coordinate care and make referrals to specialists as appropriate, • Participate in care management of high-risk patients to reduce hospital and emergency department utilization, • Collaborate with interdisciplinary teams to support population health and quality initiatives, • Supervise and provide clinical guidance to assigned clinical staff, residents, or students as applicable, • Maintain timely, accurate, and complete documentation in the electronic health record, • Participate in departmental meetings, training, and quality improvement activities, • Ensure compliance with federal, state, and organizational regulations including HIPAA, OSHA, and CMS/FQHC requirements, • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school, • Completion of an accredited residency program in an appropriate specialty, • Board Certified or Board Eligible (must obtain certification within required timeframe), • Current and unrestricted Illinois Physician and Surgeon license, • Active DEA registration (Federal), • Active Illinois Controlled Substance Registration, • Two (2) or more years of clinical practice experience in outpatient, hospital, or private practice settings, • Experience working in community health, FQHC, or underserved populations, • Proficiency with electronic health records (Centricity or comparable system preferred), • Clinical setting with extensive direct patient contact, • Exposure to communicable diseases, including tuberculosis, • Ability to sit, stand, walk, bend, and lift up to 25 pounds as needed, • Maintain patient access and productivity by averaging 18–22 patient visits per clinic day, measured monthly and reviewed quarterly., • Ensure timely and compliant documentation by completing 100% of patient encounter notes within 48 hours of each visit. Employees in this role are required to comply with annual TB testing as part of workplace health and safety protocols. The only acceptable exceptions to this requirement are documented medical contraindications or religious objections.
