Job Summary: Partner Success Manager – Remote Care (Clinic Integration/ Provider-Facing) Brook is seeking a Partner Success Manager to lead the clinical and operational success of our Remote Care programs. In this role, you’ll support provider groups and clinic teams as they adopt Remote Patient Monitoring (RPM) to improve chronic condition management, patient engagement, and reimbursable care delivery. You’ll serve as the liaison between designated partners and internal teams: guiding onboarding, aligning workflows, tracking utilization and outcomes, and collaborating across Brook teams to remove barriers and provide seamless integration of services. Success is measured by clinic adoption, patient enrollment , quality and revenue performance, and long-term partner engagement. Requirements: Key Responsibilities: Clinic and Partner Relationship Management • Serve as the primary day-to-day contact for assigned clinics; build trusted relationships with physicians, care coordinators, and admin leaders, • Understand each clinic’s workflow (EHR documentation, staffing, communication, billing, etc. ) and tailor Remote Care adoption accordingly, • Provide training and refreshers on RPM program tasks, patient enrollment, and documentation for billing compliance, • Run regular partnership reviews: outcomes, growth, operational efficiencies , and develop action plansImplementation, Adoption & Workflow Alignment, • Lead (or co-lead with a Senior Partner Success Manager) onboarding and go-live milestones for new partners, • Assist with facilitating integration between partner EMR/EHR workflows and Brook’s platform., • Track activation, engagement, and reimbursement metrics via CRM and performance dashboards; escalate risks early, • Collaborate with Clinical Operations, Product, and Revenue Cycle to resolve data gaps, billing issues, or workflow frictionAccount Performance & Operational Impact, • Maintain a success plan for each partner: adoption goals, RPM utilization targets, patient cohorts, and operational workflows, • Identify opportunities to improve adoption, documentation accuracy, health outcomes, and reimbursement efficiency, • Analyze performance trends and prepare partner reports Required • Bachelor’s degree in Healthcare Administration, Public Health, Nursing, Business, or a related field, • 2–4 years of professional experience that combines:, • Healthcare exposure: Experience supporting or working alongside provider groups, clinic operations, care coordination, or population health, • Customer/Partner success or implementation: Experience in digital health or technology-enabled services involving account or partner management, • Familiarity with Remote Patient Monitoring (RPM), Chronic Care Management (CCM),, • Working knowledge of EHR/EMR usage (e.g., documentation, workflows, reporting, integration), • Proficiency with CRM tools and performance dashboards, • Strong communication, problem-solving, and relationship-building skills, • Able to manage multiple Provider accounts or projects with guidance from a Senior Partner Success ManagerPreferred, • Experience supporting technology adoption within ambulatory clinics, health systems, or multi-site provider networks, • Understanding of CPT codes, claim submission, and value-based care programs, • Background in care quality programs, population health, or chronic care coordination, • Hybrid role based in Derby, CT, with 3–4 days available per week on-site at partner clinics/hospital or in-office, • Local/regional travel as needed to support partner training and relationship-building, • High-Impact Role: Own a core growth channel in a sector of healthtech experiencing rapid growth., • Mission-Driven: Join a company that’s improving patient outcomes and transforming the way healthcare is delivered., • Fast-Growing Environment: Thrive in a dynamic, scale-up atmosphere where you’ll have the autonomy to create meaningful change and implement new ideas from day one., • Cutting-Edge Tech: Work with innovative tools and have the freedom to introduce new solutions, including AI-driven approaches., • Fast-paced environment – Brook operates in two of the fastest changing industries in America – Healthcare and Technology.
