Job Description Job description: Nurse Home Visitor - Nurse Family Partnership Full-Time | 33.5 hrs/week | $32.72-$34.44/hour Are you a nurse looking for meaningful work—without shift work, overtime, or burnout? Join the Nurse-Family Partnership (NFP) team at CIDS and use your nursing skills to support first-time parents and their children during the most important early years of life. As a Nurse Home Visitor, you’ll develop strong, long-term relationships with up to 24 families, delivering evidence-based care through home visits from early pregnancy until a child’s second birthday. You’ll also participate in hospital-based outreach as part of the Infant Registry team, connecting with new families on Arnot Hospital’s Labor and Delivery unit to offer congratulations, share information about CIDS programs and community resources. Hospital coverage is shared on a rotating schedule, typically 1–2 weekdays per week and 1–2 weekends (up to 4 hours) per month. This role combines clinical nursing, education, and public health in a supportive, family-friendly environment—offering flexibility, professional growth, and the opportunity to practice nursing in a truly impactful way. Job Duties: Nurse Home Visitors will • Provide home visits up to 25 families in accordance with NFP model., • Utilize screening and assessment tools to partner with participants and empower them to create goals., • Develop and maintain therapeutic relationships through reflection and motivational interviewing to effectively provide education, support, and referral resources., • Participate in community outreach, and collaboration with other service providers in Chemung and surrounding counties to ensure continuum of care., • Complete all required training for professional development., • Participate in hospital-based outreach on a rotating schedule as part of the Infant Registry team., • Maintain accurate and timely documentation according to program, agency, and state requirements. Qualifications: Education/Experience: Bachelor of Science in Nursing (BSN), RN license (in good standing). At least two-years of experience with home visiting, maternal-child health, public health, or mental/behavioral nursing preferred. Must have excellent written, verbal communication skills and basic computer skills Occasional travel for in-person training, and continuing education may be required. Must have the ability to lift at least 25 pounds, have an active driver license and vehicle. Must be able to work flexible hours, including days, evenings, and occasional short weekend hours.
