Pamir Homecare is currently seeking a dedicated and experienced Care Coordinator/Field Supervisor to join our team in Ealing and Harrow. In this vital role, you will be responsible for supporting and monitoring the performance of domiciliary care workers, ensuring the smooth setup of new care packages, and maintaining high standards of client satisfaction. You will play a key part in creating and supervising optimised staff schedules that meet both client needs and workforce capacity. Working closely with the Registered Manager, you will also conduct field supervisions, competency assessments, and contribute to the ongoing professional development of care staff.

Working from the guidelines from the 1989 Children’s Act, National Minimum Standards, Every Child Matters and any other relevant legislation.  To provide a caring, supportive and nurturing environment in which children/young people can feel secure and free from harm.  Establishing positive relationships with young people and always offering them unconditional and positive regard.  Helping young people gain self control by challenging unacceptable behaviour and rewarding acceptable, pro-social conduct.  Ensuring that each young person’s care plan is followed and amended as appropriate to reflect their changing needs  Attending to practical matters in relation to childcare (cooking, cleaning, general maintenance around the home etc)  To act as a key worker or co worker for a young person to ensure that the young person’s care plans are up to date and that all their care needs are being met. To provide a positive role model to be able to offer advice, guidance and assistance where appropriate.  Establishing relationships which young people perceive to be positive, warm and rewarding  Providing advice, assistance and support on a 1:1 basis to enable young people to address past and present difficulties  Providing emotional support at times of difficulty or stress

Job Summary We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our healthcare team. The ideal candidate will possess a strong background in patient care, with experience primary care. This role involves collaborating with healthcare professionals to ensure the safe and effective use of medications, providing expert advice on medication administration, and contributing to the overall wellbeing of patients. Responsibilities Review and interpret medication orders for accuracy and appropriateness. Provide clinical consultations to healthcare staff regarding medication therapy management. Monitor patient progress and medication effects, adjusting treatment plans as necessary. Educate patients and their families about medications, potential side effects, and proper administration techniques. Collaborate with multidisciplinary teams to optimise patient care outcomes. Maintain up-to-date knowledge of pharmacotherapy, including anatomy knowledge relevant to medication use. Participate in quality improvement initiatives related to medication safety and efficacy. Qualifications Bachelor’s or Doctor of Pharmacy degree from an accredited institution. Valid pharmacy licence to practice in the relevant jurisdiction. Previous experience in a PCN setting is highly desirable. Strong understanding of patient care principles and medication administration protocols. Excellent communication skills, both verbal and written, with the ability to work effectively within a team environment.
