Key Responsibilities Hospital & Community Coordination Build and maintain excellent working relationships with Great Western Hospital, community healthcare teams, and local partner organisations. Coordinate referrals from hospitals, healthcare professionals, and community services to ensure safe and timely care at home. Match carers with customers based on skills, experience, and individual needs. Manage last-minute changes, emergency cover, and staff absences. Act as a key point of contact for customers, families, and care staff. Respond promptly to telephone calls, emails, and messages. Record and escalate concerns, complaints, compliments, and safeguarding issues appropriately. Work closely with the Registered Manager and wider care team to maintain high standards of service quality. Participate in the shared out-of-hours on-call rota, providing support to customers and staff while managing emergencies and operational queries.About You We are looking for someone who has: A minimum of 1 year's hands-on care experience (essential). Experience working within domiciliary care, community services, hospital discharge, or care coordination (desirable) Excellent communication skills and the confidence to build professional relationships across health and social care. Strong organisational and problem-solving abilities. Experience in rota planning or scheduling (desirable). Confidence using care management or rostering systems (training provided if required).
