Job Summary and Qualifications The Quality Coordinator - Sepsis assists with the coordination of the Performance Improvement Program to ensure high quality, patient-centered care by supporting the daily operations of the Quality Management/Patient Safety Program(s) and Performance Improvement programs. The Quality Coordinator collaborates with the Quality Leadership Team and other stakeholders to monitor processes, identify opportunities, obtain analyses, provide constructive input and channeling concerns appropriately, in compliance with applicable laws, regulations, and other governmental requirements. The Quality Coordinator - Sepsis highlights priorities through data analysis and trending, incident response, and assists with staff education to advance patient safety and control and minimize risks at the facility. About this Unit: • Work Schedule: Monday - Friday (8am- 4:30pm), • Ensures compliance with all hospital policies and procedures, safety and emergency preparedness requirements, Joint Commission core measures initiatives, accreditation requisites, and federal, state, and local regulations., • Willing to adjust personal schedule in cases of workload fluctuations or other departmental needs., • Demonstrates effective verbal & written communication., • Responds to supervisor’s & co-workers requests for information & assistance in a cooperative manner & within appropriate time frame, • Interacts and communicates in a way that promotes a harmonious and cooperative working environment., • Communicates problems, workflow, opportunities for improvement and other relevant issues to supervisor or others as appropriate., • Demonstrates respect for others’ opinion, judgement, capabilities and gives praise to those who earn it., • Consistently displays behavior that enhances the image of the organization as evidenced by professional and courteous behavior toward all customers including co-workers., • Continuously improves service outcomes that impact patients and customers by awareness of the process design, measurement, assessment and commitment to improvement that is consistent with the hospital mission, vision and scope of service statements, with a commitment to excellence., • Supports and participates in the organizations’ Continuous Quality Improvement Program. Demonstrates participation in the Care Council., • Anticipates work flow, prioritizes tasks, and meets deadlines., • Accumulates data, forwards findings and results to responsible personnel., • Executes preparation / presentation of reports., • Reviews medical record per approved criteria., • Determines need for trending and/or more intensive review., • Ensures that appropriate follow-up is conducted., • Ensures the evidence based screening and treatment protocols become the standard of care within the facility., • Collaborates with medical and hospital staff to provide multidisciplinary care to patients with a diagnosis of sepsis, severe sepsis and septic shock., • Reviews and analyzes clinical data and presents the data in an understandable and meaningful context to staff and physicians to motivate adherence to protocols., • Observes clinical processes and conducts surveillance activities, with immediate intervention (just-in-time) as indicated., • Forms idea for process improvement, develops and engages team solutions., • Provides education and clinical expertise to care team (physicians, LIPs and staff), patients and families., • Collaborates with physicians and staff in an effort to maximize timely evidence based care to decrease morbidity, mortality, length of stay, and readmissions., • Leads sepsis committee meetings with physician and nursing champions., • Leads hospital, medical staff and shared governance meetings focusing on sepsis as needed., • Participates in multidisciplinary rounds and proactively responds to patient needs to improve patient outcomes and positively impact overall patient experience., • Works in collaboration with the care team to develop and implement individualized teaching plans for patients and families., • Collects and analyzes sepsis program data to drive the program., • Participates in hospital, division and corporate wide collaboration for sepsis care and treatment., • Maintain knowledge and understanding of changing performance measurements for high quality patient outcomes., • Locates, accesses, and understands local and national best practice benchmarks related to patient experience and outcomes., • Graduate of an accredited School of Nursing., • Current Registered Nurse License in Kansas or Current Multistate Compact License., • 2 years of current nursing experience in an acute care hospital setting.
