Position Overview The Home Health Aide (HHA) provides essential personal care and support services to clients in their homes under the supervision of a Registered Nurse or Nurse Supervisor. The HHA’s tasks are non-invasive, focused on daily living assistance, and must align with the nursing care plan. The role does not include the administration of medications or the performance of clinical procedures requiring licensure. HHAs may include Certified Nursing Assistants (CNAs), Licensed Practical Nurses (LVNs), or RNs providing aide-level care under this scope. Essential Duties And Responsibilities DUTIES * Provides personal care and related services in the home, under the direction, instruction and supervision of the staff nurse and the Nurse Supervisor * Tasks to be performed by a HHA must be assigned by, and performed under, the supervision of an RN who will be responsible for the patient care provided by the HHA. * Home Health Aide will not administer medications, take physician’s orders, receive, or reduce any intravenous procedures, or any other sterile or invasive procedures, other than rectal temperatures or enemas, or perform procedures requiring the training, knowledge, and skill of a licensed nurse. * Home Health Aide services may also be provided by a CNA (Certified Nursing Assistant) subject to the same supervision described below. If necessary, they may also be provided by an LVN or RN. RESPONSIBILITES * Follows the plan of care to provide, safe, competent care to the client * Helps the patient to maintain good personal hygiene and assists in maintaining a healthful, safe environmen * Plans, and prepares, nutritious meals, grocery shops, when instructed to do so, by the nurse * Assists the patient with ambulation, as ordered, by the physician, and approved, and supervised, by the nurse * Assists the therapy personnel, as needed, with rehabilitative processes * Encourages the patient to become as independent as possible, according to the nursing care plan * Attempts to promote patient's mental alertness, through involvement in activities of interest * Gives simple emotional, and psychological, support to the patient, and other members of the household, and establishes a relationship with patient and family, which transmit trust and confidentiality * Reports any change in the patient's mental, or physical, condition, or in the home situation, to the staff nurse, or to the Aide supervisor * Performs routine housekeeping tasks, as related to a safe and comfortable environment for the patient, as instructed by the professional nurse * Prepares a visit report, promptly, and incorporates same in the clinical record, weekly * Confirms, on a weekly basis, the scheduling of visits so that other necessary visits, by staff members, can be coordinated * Works with personnel of other community agencies involved in the patient's care, as directed by the nurse * Attends in-service, as required by regulation or Agency policy * Has access to all client medical records which may be discussed with the Registered Nurse and the Nurse Supervisor Job Conditions * The ability to drive and the ability to access clients’ homes which may not be routinely wheelchair accessible are required. * Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client’s condition and to perform client care. * On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift and/or carry up to 50 pounds. * Must be able to communicate clearly, both verbally and in writing in English. * Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. Qualifications * High school diploma or equivalent * Must have completed a minimum of 75 hours of training, which includes an introduction to personal care services * If a Nursing Assistant, certified by the state, (CNA), the individual will meet the qualifications for a Home Health Aide, as his or her hours of training will exceed the minimum required. * Must understand, and respect, clients, including ethics and confidentiality of care * Must have current CPR certification. Online certification is not accepted. * Must have criminal background check * Must be free from health problems that may be injurious to patient, self, and co-workers and must present appropriate evidence to substantiate this * Must comprehend the basics of personal care, housekeeping, and meal preparation * The ability to communicate well, both, verbally, and in writing, in English, is required. Environment * Contact with clients under a wide variety of circumstances. * Care is provided in client’s living environment with varying situations, including private homes, independent or assisted living or in an institution such as a hospital or long-term care facility. * May be required to respond to emergency situations. * Position includes tasks that have the potential for exposure to blood/body fluids/tissues and other potentially infectious material such as body fluids/tissues. * Risk Of Exposure To Bloodborne Pathogens – High Physical Requirements * Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. * Must be able to lift up to 50 lbs. * Corrected visual and hearing acuity required. Other Requirements * Must be able to provide two professional references * Must pass medical clearance and background check * Must be able to communicate clearly and effectively in verbal and written English Work Hours Work hours will vary depending on the client's needs. Work hours may include day, evening, weekend, or holiday shifts, based on client needs and the availability of the employee.
