About Superior Hospice Superior Hospice is a growing, locally based hospice provider serving patients across a 21-county service area throughout the Twin Cities and surrounding communities. Our team focuses exclusively on hospice care, providing compassionate, patient-centered support wherever our patients call home. We are expanding quickly and looking for an experienced RN who wants a high-impact, flexible clinical role supporting admissions, referrals, visits, and case management across the organization. This is an ideal position for a nurse who enjoys variety, autonomy, and working at the top of their license while still being part of a strong, supportive team. What You’ll Do • Review referral lists, SHP reports, and intake updates to identify potential hospice patients., • Follow up with hospitals, SNFs, and senior living communities for informational visits., • Work closely with intake to prioritize referrals and schedule admissions and informationals., • Complete hospice admissions including eligibility assessment, consents, education, and start of care., • Provide visit nurse coverage when case managers are unavailable or census is high., • Communicate with discharge planners, social workers, and providers regarding hospice eligibility., • Help grow census by maintaining strong relationships with referral sources., • Document accurately and follow Medicare hospice regulations., • Step in wherever clinical support is needed to keep patient care running smoothly. What We’re Looking For • Active Minnesota RN license required., • Hospice experience preferred OR strong background in ICU, oncology, palliative care, or high-acuity nursing., • Experience with hospice admissions strongly preferred., • Comfortable working independently in the field., • Strong communication and relationship-building skills., • Organized, flexible, and able to adapt to changing priorities., • Reliable vehicle and ability to travel throughout the service area.
