Job Responsibilities: Job Summary Supports the Bakari Center Team with monitoring the physical and mental health needs of the youth residing in the Residential Care Center. Essential Duties and Responsibilities • Set-up weekly medications and monitor need for medication refills., • Review the medication record for accuracy and possible errors., • Meet with youth 1:1 during walk-in times to assess youth’s physical and mental health needs., • Assist with scheduling medical appointments to follow-up., • Accompany youth to appointments to discuss medical needs as needed., • Support Bakari Center staff with medication and medical questions., • Provide follow-up consultation with medical providers regarding medical concerns., • Serve as liaison with other collaborative medical professionals when necessary., • Instruct youth and their caregivers about medications, side effects, and precautions., • Document any youth or collateral contacts in the electronic record., • Review and support the implementation plan for youth medications., • Sign off for staff who successfully complete CBRF medication administration training., • Enter new youth medication orders and all medication changes in the MAR electronic record as needed., • Facilitate health education group with youth., • Alert manager of any medical supplies that need to be ordered., • Attend and participate in staffing, team meetings, and trainings as needed., • Other job-related duties may be necessary to carry out the responsibilities of the position. Job Qualifications: Required Qualifications • Have a 2-year degree from an accredited Nursing School., • Registered Nurse licensure., • 1-2 years nursing experience with youth., • Provide valid driver’s license, automobile, and insurance sufficient to meet agency requirements., • Meet all the employee requirements including references, criminal background check, drug test, and driver’s license check Knowledge, Skills and Abilities • Communication – Ability to provide information effectively and in a positive manner with a diverse population - the persons served, supervisor, colleagues, and program partners in writing and oral communications., • Technological Aptitude – Ability to use general technological skills throughout daily job i.e., Email, Internet, company specified systems, (ex. Microsoft 365, Windows, Word, Excel)., • Managing Priorities/Deadlines – Ability to maintain schedules, meet deadlines and manage multiple projects. Possess skills for effective use of time and resources to help maintain quality services., • Problem-Solving Skills – Ability to think critically and be solution-oriented in a fast-paced environment and adapt to program changes and challenges., • Adaptability – Ability to manage change, deal with situations as they arise, respond appropriately in crisis situations, and work independently or as part of a team., • Teamwork – Ability to work as a team participate productively while also managing independent contributing duties and responsibilities. Ability to maintain and promote positive working relationships within the residential care center, • Motivation – Possess a commitment to the assigned job, mission and core values of the organization while also supporting a respectful and harmonious work setting., • Professionalism – Ability to conduct oneself with a high level of integrity, ethics and boundaries. Must demonstrate professionalism, cultural sensitivity, and strong interpersonal skills while representing the organization in a variety of settings., • Healthcare Knowledge – Possess good clinical skills especially related to medical issues, mental health, and substance use disorder. Demonstrate knowledge of psychotropic medications and their side effects and have ability to assist youth with medical needs., • Clients’ Rights – Demonstrate skills of clients’ rights including principles of ethics and confidentiality. pm21 Other Job Information (if applicable): PHYSICAL DEMANDS: Position is mobile within the Residential Care setting; most of the work is done standing or sitting at a desk or counter and using the computer; must be able to go up and downstairs and lift and carry 20 pounds. Requires high level of concentration and the ability to accurately read computer screens.
