Home management tasks include housekeeping, laundry (may be required to iron a piece of clothing or sew a button), shopping and errands., • In performance of essential shopping and running errands must provide the necessary receipts and money transaction documentation to participant and agency., • Prepare and serve meals and/or simple modified diets according to instruction and assist with feeding as necessary., • Personal care tasks for participants that are not medical in nature, such as, personal grooming, shaving, hair shampooing and combing, bathing to include sponge bath, shower bath or tub bath, toileting/use of bedpan, commode or toilet, assistance with getting dressed, nail care (filing), respiratory services according to policy, brushing and cleaning teeth or dentures., • Assist with lifting participant out of bed, positioning/transferring participant and ambulation including use of cane, walker, and wheelchair or other use of specific adaptive equipment., • Assist self-directing participant with medication reminders or refilling of prescription at local pharmacy as specified in plan of care., • Provide routine skin care that includes ensuring skin is properly clean and dried after bathing and/or incontinence circumstances. Assist with the change of disposable diapers., • Assist self-directing participants in reminding them of their home maintenance exercise/range of motion programs., • Adherence to the Electronic Verification Visit policy. Guided by low-barrier, housing placement principles, the RHI Program eliminates preconditions such as abstinence or mandatory treatment participation, offering immediate access to housing. Facilitates quarterly file audits., • Continually monitors performance of assigned personnel and reviews performance with individual employees per One Family Illinois guidelines., • Conducts monthly team meetings; attends and participates in agency meetings, committees, work groups, etc., • Input staff employee hours and timecard details into payroll system; approve expense reports and timecards by established deadlines., • Responsible for identifying, cultivating, and maintaining community resources, referral options, and relationships with service providers, organizations, and other resources and educate the team regarding effective service delivery., • Supervise, coordinate, and participate in outreach activities with schools, youth-service agencies, therapists, doctors, referral sources, court personnel, neighbors, churches, etc. Act as a liaison to the community and for all external stakeholders., • Reviews all program data; develops programmatic short-term and long-term goals; identifies and develops quality improvement projects that support the mission of Casa Tepeyac; prepares and submits reports to agency leadership and other stakeholders as required., • Establishes and maintains an environment that promotes integrity and accountability; innovation and collaboration; and a commitment to providing the highest quality of care possible., • Performs on-call and all other duties as needed and assigned. Job Description SUMMARY: Participates in an on-call schedule to provide crisis intervention to clients, assessing for inpatient mental health hospitalization. Crisis services are provided within the community, on-site of the crisis. For on-call shifts, the After-Hours Counselor must provide on-site assessment within 60 minutes. PCH is centralized in Berwyn, but staff travel north up to Oak Park, East to Cicero, South to Justice and West to La Grange. Most assessments on the weekend are at one of our 4 Emergency Rooms: West Suburban, Rush Oak Park, LaGrange Memorial and MacNeal. ESSENTIAL DUTIES AND RESPONSIBILITIES • Provide crisis intervention services in person or by telephone to police, school and hospital personnel, and community residents dealing with crisis situations. This includes referrals as well as diagnostic assessment and crisis intervention services., • Provide on-site assessment within 60 minutes., • Conduct emergency on-site assessments of clients to determine the need for inpatient psychiatric hospitalization., • Assess and stabilize high risk individuals and/or families., • Completes required documentation and maintains records in accordance with PCH policies and procedures., • Participate in departmental meetings and completes all required training requirements., • Meets with supervisor at least once per month to discuss crisis situations. Our approach is unique, and our care is excellent. About the Position We are hiring DSPs who are professional, energetic, and nurturing. You will provide hands-on support to individuals in their homes as they work on daily activities and long-term goals. This includes activities such as meal preparation, enhancing social skills, providing companionship, community outings, and more. Location & Schedule Note: Your shift will not vary and you must be able to work the entire shift, every week.
