Job DescriptionPOSITION SUMMARY The Integrated Medicine and Wellness Program Coordinator is a key administrative role supporting the strategic coordination of HHM Health’s integrated care model, with a primary focus on enhancing Adult Medicine services. This position helps align operations across Internal Medicine, Wellness, Behavioral Health, Nutrition, Pharmacy, Vision, and OB/GYN to ensure a seamless and comprehensive patient experience. The coordinator plays a critical role in supporting care delivery, administrative planning, and stakeholder engagement to advance whole-person care. This role also serves as the central liaison for external providers and agencies seeking to partner with HHM’s integrated Adult Medicine services. KEY RESPONSIBILITIESAdult Medicine Integration and Service Coordination • Drive the alignment of Internal Medicine workflows with Pharmacy, Behavioral Health, Dental, Nutrition, Vision, Imaging, Psychiatry, OB/GYN, and other ancillary services to enhance coordinated care for adult patients., • Support Adult Medicine providers in navigating internal referral processes and care pathways across service lines., • Maintain integration tools such as cross-departmental protocols, care coordination maps, service line directories, and orientation materials.Operational & Programmatic Support, • Assist in the administrative planning, implementation, and ongoing support of wellness and chronic disease programs focused on adult populations (e.g., diabetes, hypertension, weight management)., • Collaborate with department leaders to develop patient engagement strategies, educational resources, and multidisciplinary care initiatives that improve Adult Medicine access and outcomes., • Track and help analyze operational metrics related to integrated adult services, including provider participation, referral volumes, and service utilization., • Coordinate the schedules and activities of Community Health Workers and Educational Specialists to ensure alignment with Adult Medicine and wellness initiatives.New Service Lines and Optimization, • Coordinate the integration of new services, staff, or programs into the Adult Medicine ecosystem, including provider onboarding, clinical workflow mapping, and communications., • Ensure all new initiatives align with HHM’s integrated care philosophy and support whole-person care for adult patients., • Facilitate collaborative planning between Adult Medicine and supportive services.External Liaison and Stakeholder Relations, • Act as the primary point of contact for external agencies, residency programs, and community providers seeking to collaborate on Adult Medicine and integrated services at HHM Health., • Organize informational sessions, tours, and partnership onboarding in coordination with leadership and clinical departments., • Maintain tracking systems for outreach activities, partnership timelines, and provider collaboration outcomes.Communications and Interdepartmental Collaboration, • Coordinate internal meetings, integrated care committee updates, and strategic communication between departments supporting adult patients., • Collaborate with IT, quality improvement, and communications teams to standardize documentation, reporting, and program promotion., • Contribute to internal education initiatives that train staff on integrated workflows and best practices for whole-person care.Qualifications, • Bachelor’s degree in healthcare administration, Public Health, Social Work, or a related field preferred, but not required., • Minimum of 2 years of administrative experience in healthcare, with a strong preference for experience supporting adult or internal medicine services preferred., • Prior experience working in integrated or multidisciplinary care models is strongly preferred., • Experience with integrated care delivery within a Federally Qualified Health Center., • Understanding of value-based care principles, HEDIS, and UDS reporting, particularly as they relate to adult health metrics., • Familiarity with behavioral health integration, medication adherence programs, nutrition services, and vision care in primary care settings.Skills and Competencies, • Strong coordination, organization, and communication skills across clinical and administrative teams., • Knowledge of healthcare delivery systems with a focus on Adult Medicine and chronic care management., • Familiarity with eClinicalWorks, Excel, Microsoft Office, Teams, and healthcare quality reporting.
