Job Description ST. COLETTA OF GREATER WASHINGTON CLASS SPECIFICATION ADAPTIVE PE TEACHER Definition Under the direction of the School Systems Specialist, the incumbent is responsible for designing and implementing age-appropriate motor/leisure programs for students with intellectual disabilities, autism, and/or multiple disabilities. Duties include providing quality adapted physical education activities with appropriate accommodations, modifications, and adaptations to make each activity accessible to students and to improve their gross motor skills. Taking into consideration each student’s individual communication needs is essential. Train classroom staff to support their students during physical education activities. Essential duties require the ability to keep up with running students and or to lift or assist with lifting a student. An important aspect of the job is gaining general knowledge of and implementing the assigned student’s Individual Education Plan (IEP) goals and objectives. Job Description Rapid Re-Housing Case Manager The RRH Case Manager will provide the support services needed to ensure that program participants succeed in retaining their housing. Emphasis is placed on regular home visits and utilizing programs and benefits designed to increase income, financial stability, and quality of life. RRH will assess participants to determine eligibility for rapid re-housing assistance. The RRH Case Manager will work closely with the Housing Locator in identifying suitable housing for program participants. Since the contract in place mandates direct service to clients, this position is required to report to the PathForward office 40 hours a week. Responsibilities Include: • Assist in identifying and engaging eligible program participants for the RRH program, • Complete initial referral application with prospective program participants and all requisite paperwork to the CRT (Case Review Team) with all required documentation attached, • Complete a comprehensive assessment with program participants who have been accepted to the RRH program. The assessment must be completed within three days of acceptance into the program, • Report on program participant challenges, successes and transitions (Case Conferencing conducted through CCU), • Meet with the participant within 3 days of receiving the case assignment, • Develop a Housing Services Plan with each program participant. Position Summary: The Coordinator, Fundraising Campaigns & Volunteer Engagement is an important fundraising position that works in partnership with the Chapter's Executive Director, leadership volunteers, and committees to drive growth and success in fundraising campaigns. This individual will raise money from donors and event participants, and identify, recruit, train, and partner with leadership volunteers to harness their passion and skills and to help maximize their engagement in order to advance the mission of the Foundation. The Coordinator travels throughout the market area to meet with constituents and serves as a key contact for the Foundation with volunteers and supporters. The Coordinator will help identify and cultivate partnerships and alliances that will promote growth and awareness in the community. This is a hybrid role with 2 days/week in the Bethesda, MD office and 3 days remote work. Essential Functions & Responsibilities: Fundraising • Develop impactful relationships within the patient and corporate community empowering them to tell their stories through direct peer-to-peer fundraising., • Go the extra mile to understand the full scope of these diseases and in turn, use that knowledge to connect with patients in a meaningful way. Communicate shared vision by utilizing national campaign and marketing materials, adjusting as appropriate., • Execute on annual operational plan including marketing, budgeting, and project management to ensure success of the Take Steps and Special event campaigns., • Ensure revenue goal is achieved by developing, implementing, and assessing year-round strategic fundraising plans for all events held in the DC, Maryland, and Virginia market areas. Plans includes campaign revenue drivers: proactive team engagement, acquisition, day of experience, sponsorship and data-revenue tracking and assessment, coordinating with lead staff to make timely adjustments to fundraising strategies as needed., • Prioritize relationship development. Job DescriptionSalary: 45-50K Case Manager LGBTQIA+ Shelter Reports to: Program Manager / Shelter Coordinator Location: KBEC Group, Inc., Washington, DC Position Summary The Case Manager is responsible for providing person-centered, trauma-informed case management services to individuals and families experiencing homelessness, with a focus on clients housed in KBEC Groups LGBTQIA+ Low-Barrier Shelter and related housing programs. The primary goal is to support clients in overcoming barriers to permanent housing using a strengths-based, Housing First approach. This role includes serving as a CAHP (Coordinated Assessment and Housing Placement) Coordinator for DC voucher programs, ensuring timely housing matches, voucher utilization, and coordination with landlords, housing providers, and community partners. Essential Duties and Responsibilities Client Support & Case Management • Provide comprehensive, client-centered case management to individuals in shelter and supportive housing programs., • Deliver supportive services to clients experiencing homelessness, including those with serious mental illness, substance use disorders, chronic health conditions, and co-occurring challenges., • Conduct regular face-to-face contacts, spending approximately 80% of time directly engaging with clients., • Complete home visits, shelter check-ins, and safety/wellness assessments., • Develop, implement, and review individualized housing and service plans focused on stability, independence, and long-term housing retention., • Provide budgeting, financial literacy, and Representative Payee services as needed., • Support clients in accessing medical, psychiatric, and community-based services., • Oversee medication management for assigned clients, when required. Housing & CAHP Coordination • Serve as a CAHP Coordinator for DC housing voucher programs, ensuring clients are matched to appropriate housing opportunities., • Assist clients with housing applications, voucher utilization, and landlord engagement., • Conduct housing inspections and monitor rental payments to ensure compliance with program requirements., • Act as a liaison with landlords, housing providers, and community stakeholders to support housing stability. Compliance & Documentation • Complete all required documentation in HMIS and other databases, including case notes, assessments, and service plans., • Administer VI-SPDAT, F-SPDAT, and/or TAY-SPDAT assessments as part of Coordinated Entry., • Maintain accurate, timely, and audit-ready records in accordance with HUD, DHS, and KBEC Group standards., • Collect outcome data and prepare monthly progress reports.
