Management • With the Clinical Director, ensure clinicians are accountable for their documentation.., • Manage clinicians timeliness for encounters weekly and quarterly., • Meet with clinicians that are not making contracted hours due to documentation concerns and create plans for improvement., • Partner with the administrative medical records team to receive and track all records requests entering CCCC., • Review all records releases to determine whether release is required/advisable, and that required documentation is included., • Oversight of medical records policy implementation which dictates how different classes of medical record requests are handled, e.g. client requests, provider requests, disability requests, insurance requests, insurance audits., • The LC-MR implements and changes SOPs for encounter management, medical records releases, and medical records audits to align with insurance documentation standards., • If the request is an insurance audit, the LC-MR will work with the administrative team to ensure that Senior Leadership and all affected clinicians are aware., • Work with the administrative team to manage the communication with affected clinicians and manage any addendums needed., • Once affected clinicians approve their documentation, the LC-MR will review the records an additional time and compare to the request and provider manuals as needed and send them back to the clinician as needed., • Once approved by the LC-MR, the LC-MR will forward the request to the Clinical Director for final approval., • Once approved by the Clinical Director, the LC-MR will manage the administrative team to release the records to the requesting agency or person., • Use the internal tracking system to ensure turnaround meets CCCC Audit Standards., • Oversight of standardized medical record and documentation policies and procedures and timely implementation., • Oversight of activities of all administrative and clinical staff working on medical record and documentation initiatives., • The LC-MR is responsible for the ongoing internal auditing of clinicians at CCCC., • The LC-MR will develop and maintain a tracking system to ensure that all clinicians are audited twice a year and any amendments to records are completed within the prescribed timeframes below., • Twice a year, the LC-MR will review two full records for each 1099 and W2 clinician CCCC., • The LC-MR will randomly review the full records for 12 clinicians a month (3 a week) to ensure compliance with current clinical documentation standards and CCCC Checklist., • The LC-MR will communicate with clinicians whose records do not meet requirements and ensure corrective actions are implemented and completed within one month of review., • Work with Senior Leadership for clinicians who fail to meet documentation requirements.Meetings, • Attend the monthly managers meeting and report on the areas they manage., • Meet with clinicians undergoing requests or audits to review documentation as needed., • Meet with clinicians undergoing internal auditing whose documentation does not meet clinical standards., • Lead one clinician meeting per week., • Working with outside consultants as needed, the LC-MR will facilitate annual medical records training for clinicians at CCCC., • Working with outside consultants as needed, the LC-MR will develop clinical documentation standards for CCCC and update them as clinical standards change., • Provide onboarding training for new CCCC clinicians in medical record documentation., • Hold ongoing training sessions for clinicians to ensure training for standards of documentation are available regularly for clinicians., • Maintain All Clinicians database with current documentation standards and training.., • Facilitate at All Staff Meetings as requested. Working Conditions This position operates in an office setting environment. The incumbent will interact with coworkers, clients, and leadership. The incumbent may work a hybrid schedule. Physical Requirements While performing the duties of this job, the employee will frequently sit, stand, reach, and routinely answer the telephone and may spend long hours on a computer monitor. The incumbent may frequently lift 10-15 lbs. The noise level in the work environment is typically indoor and manageable. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
