Job Description Opportunity Overview Our organization is seeking a Board Eligible or Board Certified Physician specializing in Ambulatory Palliative Care. The role involves delivering Palliative Medicine consultation services and engaging with the interdisciplinary team within Palliative Supportive Care Services. Established in 2005, our rapidly expanding multi-specialty medical group comprises physicians and advanced practice clinicians operating across more than ten care centers and four hospitals in the East Bay region of San Francisco. We are affiliated with a not-for-profit health care organization that provides essential operational and administrative support, enabling our clinicians to concentrate on delivering outstanding patient care. Our group includes over 400 clinicians across more than twenty specialties, committed to offering exceptional health care to the communities we serve. • Deliver Palliative Medicine consultation services in various environments: home, skilled nursing facilities, residential care facilities for the elderly, hospitals, and offices (including Palliative Care, Cancer Center, and integrated primary care clinics)., • Collaborate with interdisciplinary team members, which include nurse practitioners, licensed clinical social workers, registered nurses, chaplains, and care coordinators., • Facilitate access to necessary supportive care services and coordinate with inpatient palliative care colleagues, referring providers, external agencies, and local hospices., • Implement clear referral criteria (e.g., excluding chronic pain management)., • Doctor of Medicine or Osteopathy., • Subspecialty Board certification or eligibility in Hospice/Palliative Medicine., • Attractive compensation package with an income guarantee., • Salary range of $262,356 to $279,077. This range represents base compensation for full-time (1.0 FTE) clinicians and does not include additional incentives or benefits.
