Community Dietitian – Band 6\n Location: Hillingdon Community Services | Central and North West London NHS Foundation Trust (CNWL) Contract Type: Bank / Agency Salary: Competitive Hourly Rate Start Date: As soon as possible \n About the Role\n We are recruiting a Band 6 Community Dietitian to join the Community Adult Rehabilitation Service within CNWL. \n This is an excellent opportunity to work as part of a supportive multidisciplinary team, delivering specialist nutritional support to adults within their own homes, care homes, and intermediate care settings. \n Key Responsibilities\n \n • \n Provide specialist dietetic assessment and treatment for adults requiring nutritional support, including Home Enteral Tube Feeding (HETF) \n\n • \n Deliver evidence-based nutritional interventions for patients who are malnourished, at risk of malnutrition, or enterally fed \n\n • \n Assess and calculate nutritional requirements, including food fortification, oral nutritional supplements, and enteral feeding regimens \n\n • \n Provide dietary counselling and support behaviour change to patients, families, and carers \n\n • \n Make recommendations to GPs regarding appropriate prescribing of oral nutritional supplements and enteral feeds \n\n • \n Participate in MDT meetings, discharge planning, and collaborative community care pathways \n\n • \n Maintain accurate clinical documentation and caseload management \n\n • \n Support training and education for healthcare staff, carers, and students \n\n • \n Contribute to audit, service development, and quality improvement initiatives \n\n\n Essential Requirements\n \n • \n Degree or postgraduate qualification in Dietetics \n\n • \n HCPC registration as a Dietitian \n\n • \n Experience working within community or adult rehabilitation settings \n\n • \n Knowledge of nutritional support, including oral nutrition support and enteral feeding \n\n • \n Strong communication, clinical reasoning, and organisational skills \n\n • \n Ability to work independently and manage a clinical caseload effectively \n\n • \n Full UK driving licence and access to a vehicle for community visits \n\n\n Desirable\n \n • \n Experience in Home Enteral Tube Feeding services \n\n • \n Experience supervising students or support staff \n\n • \n Knowledge of rehabilitation and long-term condition management \n\n • \n Experience contributing to audit or service development projects \n\n\n Why Apply?\n \n • \n Opportunity to work within a specialist community rehabilitation service \n\n • \n Supportive multidisciplinary working environment \n\n • \n Flexible bank/agency opportunity within CNWL \n\n • \n Meaningful role supporting patients to maximise independence and quality of life \n\n • \n Opportunities for continued professional development and service innovation \n\n\n Additional Information\n \n • \n Community-based role requiring travel across Hillingdon \n\n • \n Lone working involved in community settings \n\n • \n Flexible working arrangements may be available in line with service needs \n\n
