You’ll work closely with the injured person, their family, healthcare professionals, solicitors, and insurers to ensure every client receives the right support at the right time. Your responsibilities will include: • Delivering high-quality, client-centred rehabilitation services, • Writing clear, comprehensive reports and maintaining strong relationships with all stakeholders, • Managing your caseload fairly and efficiently, • Promoting evidence-based practice and innovative approaches to care, • Analysing complex clinical situations and guiding decision-making, • Monitoring progress and keeping clients engaged throughout their recovery, • Delegating effectively to your Case Manager Assistant, • Supporting colleagues and contributing to a collaborative team culture, • Assisting with onboarding and mentoring new Case Managers, • Maintaining your professional development and CPD portfolio for HCPC audit Essential Clinical Experience: • Orthopaedics, Elderly Care, MSK, Vascular & Amputations, Mental Health, Acute Medical, Community Care, Long-Term Conditions, Rehabilitation Desirable Experience: • ABI, Spinal Injuries, Paediatrics, Hands & Plastics, • Vocational Rehabilitation (supporting return to work)
