Job role: Housing & Communities Connector Team: Community Investment Responsible to: Communities Manager Hours: 5 days a week (35 hours) Salary: 34909.54 Contract: Permanent Were looking for a Housing & Communities Connector to join our Community Investment team at CHS helping our residents access the right support build on their strengths and sustain their tenancies. This is a hands-on role where youll work directly with residents build relationships with partner services and test new approaches to wellbeing and tenancy support. What youll do: • Provide one-to-one support to residents including home visits and advocacy, • Help residents navigate health social care and community services, • Co-produce personalised wellbeing and tenancy plans, • Link people to local activities groups and opportunities, • Work closely with CHS teams to address housing-related challenges About you: Youll be a natural connector with the resilience empathy and organisational skills to support people with complex needs. Youll be confident working both independently and as part of a team and comfortable visiting residents in their homes.
