    Oak Care Home Ltd is seeking reliable, caring and professional Support Workers to join its supported accommodation services for young people aged 16–18. The role involves helping young people develop the skills, confidence and independence needed for adulthood and independent living. Key responsibilities • Provide practical and emotional support., • Help young people develop independent living skills, including cooking, cleaning, budgeting, shopping and managing appointments., • Complete welfare checks and maintain appropriate communication., • Deliver planned keywork sessions in line with individual support plans., • Support education, employment, training and college attendance., • Help young people access healthcare, benefits and other professional services where required., • Promote positive routines, healthy lifestyles, appropriate behaviour and community participation., • Support young people to understand boundaries, responsibilities and house expectations., • Safeguard young people and respond appropriately to concerns, incidents and disclosures., • Follow risk assessments, support plans, safety plans, safeguarding procedures and regulatory requirements., • Complete accurate daily records, incident reports and keywork documentation., • Work collaboratively with social workers, personal advisers, education providers and other professionals., • Maintain professional boundaries and help keep the accommodation safe, clean and appropriately maintained. About you You should be caring, patient, approachable and reliable, with the ability to work effectively as part of a team. You will need to communicate clearly, remain calm in challenging situations, set appropriate professional boundaries and complete accurate written records. Flexibility is required to work evenings, weekends, bank holidays and sleep-in shifts where necessary. Essential requirements • The legal right to work in the UK., • Willingness to complete an Enhanced DBS check, including the appropriate barred-list check where applicable., • Satisfactory references and a full employment history., • An understanding of safeguarding young people., • Good written and verbal communication skills., • Basic IT skills for recording and reporting. Desirable experience and qualifications • Experience working with young people, care leavers or vulnerable adults., • Experience in supported accommodation, residential care, youth work or social care., • A Level 3 qualification in Children and Young People, Health and Social Care, or a related subject., • Knowledge of safeguarding, contextual safeguarding, exploitation and trauma-informed practice., • Experience completing keywork sessions, risk assessments and support plans., • A full UK driving licence. The Role We are looking for compassionate, reliable and experienced Support Workers / Key Workers to join our bank team and provide additional weekend cover. We are particularly looking for staff who are available across Fridays, Saturdays and Sundays, including day, evening and sleep-in shifts. Responsibilities • Support young people to develop independence and daily living skills, • Provide emotional and practical support, • Complete key-work sessions and support young people to achieve their individual goals, • Encourage engagement in education, training and employment, • Support young people with budgeting, meal planning, appointments and accessing the community, • Maintain accurate records and complete relevant documentation, • Recognise, record and report safeguarding concerns appropriately, • Work collaboratively with social workers and other professionals, • Maintain appropriate professional boundaries, • Contribute to maintaining a safe, welcoming and supportive home environment Pay & Shift Arrangements Hourly rate: £13.00 per hour Sleep-in payment: £50 per sleep-in Weekend shifts may include: • Friday: 2pm–8pm, • Saturday & Sunday: 8am–8pm day shifts, • Night shifts: 8pm–8am, • Sleep-in period: 11pm–7am, • On an 8pm–8am night shift, 8pm–11pm and 7am–8am are active working hours, paid at the normal hourly rate, with the 11pm–7am sleep-in paid at £50, • 15-minute paid handover where applicable Applicants should be flexible, as bank shifts will be offered according to the needs of the service. Main Duties • Undertake all duties and responsibilities in accordance with: company policies, procedures, protocols, guidelines, standards; good professional practice,, CQC and other regulatory requirements, relevant legislations, all legal and contractual obligations., • Provide advice, information and support to the residents., • To support and provide care for the young people/children in the home by assisting them with; bathing, eating, homework, education, behaviour, money management, travel training etc., • Ensure resident’s follow the homes rules and support the manager in investigating and taking action where these may have been breached., • Manage challenging behaviour which may require physical intervention to appropriately support the child/ young person., • Oversee the maintenance of accurate client records and databases and provide statistics for management and to assist with service evaluation in a timely fashion., • Support clients with the prompting of their medication, conduct accurate and timely medication audits and maintain accurate medication records., • Complete written documentation (support plans, Medication sheets, risk assessments) regarding the young people/ children to help and ensure their safety and development within our setting., • Take part in regular team meetings to discuss the residents, the progress that they are making, how their general well-being is as well as forming action plans to support them further., • Attend professionals meetings (where appropriate)., • Assist in the transfer of individuals between agencies and services.
