Job DescriptionSalary: $77,653 The Housing Manager is responsible for ensuring the successful implementation and management of SAVEs Housing Program. The Manager oversees all aspects of the program, including eligibility determination and intake process, distribution of rental assistance and other financial aid, development/coordination of program services such as financial literacy workshops and employability services, and training and supervision of program staff. This position works closely with the Director of Programs and the finance team to monitor the usage of grant funds and ensure compliance with funding source requirements. The Housing Manager is also responsible for developing and maintaining strong relationships with housing agencies, community non-profits and landlords in Alameda County and the surrounding areas to secure and increase permanent housing availability and supportive services for domestic violence survivors. The manager works closely with the Director of Programs and other SAVE staff to develop employment/vocational, and educational resources and opportunities for housing clients. The manager is a part of the Leadership Team and works closely and collaboratively with other managers and directors to ensure the success of SAVEs mission. Supervisory: Housing Case Managers (3) Essential Duties: • Supervise and ensure the coordination and delivery of all services of the housing program including: the eligibility, application, and enrollment processes; ongoing case management and supportive services; provision of rental payments and other client financial assistance., • Ensure compliance with funder (including CalOES and HUD) requirements by maintaining a thorough understanding of the relevant contracts., • Work closely with the finance team to monitor the direct client assistance payments and the overall budget of the program., • Work closely with the Director of Programs to evaluate and monitor the success of the program and provide strategic thinking to inform ongoing program improvements., • Develop the annual program budget., • Support fundraising/grant writing efforts to ensure the sustainability of the program., • Actively seek opportunities to deepen SAVEs relationship with partner agencies, the Alameda County Continuum of Care, local landlords and other potential allies with the goal of disseminating information and developing relationships that will contribute to the success of the housing program., • Maintain up-to-date knowledge and education related to domestic violence laws, trends, and other related topics, • Supervise, train, and support program., • Develop, monitor, and revise (as necessary) program guidelines and procedures., • Maintain a directory of housing assistance providers and related resources., • Create opportunities for volunteers to assist survivors with housing applications, searches, accompaniment, interpretation/translation, workshops, and other matters., • Oversee the maintenance of housing client files, ensuring the receipt and retention of necessary documentation related to financial assistance provided., • Submit monthly statistical reports and ensure completion of monthly and quarterly funder reports., • Provide after hours and weekend support for emergencies that may arise within the safe house or other direct service programs, responding on site as necessary. This duty is performed on a rotating basis (shared with other managers and staff), one week every 4-6 weeks and up to 12 weeks per year. (Subject to change due to staffing and Agency needs)., • Attend agency staff meetings, team meetings, and other meetings and trainings as required., • Meet with the Director of Programs for supervision regularly and as needed., • Maintain up-to-date knowledge of issues and challenges faced by survivors who are unhoused, as well as resources that may be available to them., • Maintain up-to-date knowledge of trends and laws that impact survivors of gender based violence and people who are unhoused., • Travel as required (in agency vehicle or personal vehicle) to attend meetings or trainings, transport clients, and conduct other agency business. General Duties: (shared by all direct service staff) • Respond to crisis calls and walk-in clients in a caring, informative, and non-judgmental fashion., • Maintain up-to-date files and records of all services provided., • Provide client transportation., • Attend all required meetings, including staff meetings, trainings, consultations, and retreats., • Assist with training new staff members and volunteers., • Assume responsibility for site security and safety as needed., • Provide input into program planning and development., • Assist with maintaining program operations and office functions as needed. EDUCATION AND EXPERIENCE/QUALIFICATIONS: REQUIREDKnowledge, Skill, Education/Training, and Experience Requirement(s) • Bachelor's degree in human services, social work, counseling, psychology, criminal justice, or related field., • At least 4 years of experience providing direct services at a domestic violence agency or other crisis services organization., • At least 2 years experience training and supervising direct service staff., • Must possess a valid California driver's license, insurance, and access to an automobile. Proof of insurance and loss payee endorsement is required. Job Description HERS BREAST CANCER FOUNDATION POSITION TITLE: Breast Care Specialist DEPARTMENT: Patient Programs SUPERVISED BY: Director of Programs FLSA STATUS: Non-Exempt, Full-Time & Part-Time Foundation and Position Overview HERS Breast Cancer Foundation (“the Foundation”) is seeking a full-time Breast Care Specialist (BCS), to provide breast cancer patient assessment and fitting services for post-surgical products at its Fremont, California program location. Products provided and dispensed by the Foundation include breast prosthetics, pocketed bras, and compression garments, all of which facilitate physical and emotional healing. In continuous operation since 1998, The Foundation is an invaluable resource that serves Bay Area breast cancer patients with compassion. Our goal is to restore our patients’ sense of “HERS”: Hope, Empowerment, Renewal, and Support. An accredited Durable Medical Equipment (DME) provider and a 501(c)3 nonprofit organization, the Foundation’s mission is to support all individuals healing from breast cancer by providing post-surgical products and services regardless of financial status. Mission fulfillment is dependent on year-round fundraising efforts. These include events hosted by the Foundation and by community members, grants, individual donors, and medical insurance reimbursements for products dispensed by HERS. Core Role Skills and Expectations The role of Breast Care Specialist (BCS) is central to the Foundation’s mission, programs, and services. Working in tandem with patients and their healthcare providers, allows BCS staff to advance understanding about post-surgical products, set goals and expectations, provide product use and care information, and ensure positive outcomes. Position Summary • Serve as a patient advocate during patient exam and treatment, • Show patients to examination rooms, procedure rooms or operative rooms and prepare them for the physician, • Record patient's medical history, medications, allergies, vital statistics and information such as test results in medical records, • Ensure proper consent forms are obtained prior to preparing a patient for treatment, • Prepare and administer medications as directed by a physician following safety protocol of right dose, right patient, right route, right drug and noting expiration date, • Explain treatment procedures, medications, wound care instructions and physicians instructions to patients, • Help physician examine and treat patients, handing them instruments and materials or performing tasks such as removing sutures, • Under physician direction, authorize drug refills and provide prescription information to pharmacies, • Prepare and clean exam rooms and medical equipment in between each patient and at the end of the day, • Assist physician with surgical procedures while maintaining sterile field, • Correctly dispose of infectious waste and bio hazard materials, • Schedule appointments for patients, • Inventory and assess the need for instruments, supplies and equipment while keeping the room stocked, • Performs accurate documentation of all procedures in a timely manner, • Inventory and order medical supplies monthly and ensure all equipment is in proper working order, • Adheres to all policies and procedures in performing job duties and responsibilities while supporting a culture of high quality and great customer service, • Performs other duties as needed Required Qualification (Knowledge, Skills & Abilities) Education/Training • High school diploma or equivalent, • Certified Medical Assistant, • CPR Additional Skills • Knowledge of sterile technique, anatomy, and medications, • Detail oriented, professional attitude, reliable, • Ability to follow or provide verbal & written instructions with sufficient grammar and spelling skills to avoid mistakes or misinterpretations, • Interpersonal skills to support customer service, functional, and teammate support needs, • Able to communicate effectively in English, both verbally and in writing, • Basic to intermediate computer operation, • Knowledge of state and federal regulations for this position; general understanding of HIPAA guidelines Additional Knowledge/Skills • 2+ year's experience in a medical setting preferred, • Dermatology experience a plus, • High level of emotional and professional maturity and strong interpersonal skills with the ability to build relationships and trust, navigate and flex to varied personalities and communications styles, • Unquestionable work ethic, personal integrity, and honesty, • Ability to adapt and thrive in a dynamic environment Benefits Berman Skin Institute believes our people are the secret to our success, and we work to create an environment where our people are appreciated, engaged, supported and rewarded. Job DescriptionSalary: $32.86/hr The Position: The Case Manager is a member of our Housing Team and is responsible for providing crisis intervention and case management services to survivors of Intimate Partner violence who are served in our Mariposa 2 Rapid Re-housing program. The Case Manager carries an ongoing caseload of approximately 15-20 households. The Ideal Candidate: Strong candidates for this position will demonstrate the ability to thrive in a fast-paced, crisis services environment. The right candidate will be warm and outgoing and enjoy interacting with the public. The case manager must have the ability to demonstrate empathy and compassion even in difficult circumstances. The case manager must be able to juggle multiple tasks and demonstrate skill in prioritization and organization. They must enjoy working independently as well as being part of a larger team. Previous experience providing case management services to survivors of trauma in a non-profit, direct services organization is strongly desired. Previous experience in HUD funded programs and demonstrated knowledge of HUD guidelines and requirements is strongly desired. There is a preference for candidates that speak a language (in addition to English) that is commonly spoken within the client populations we serve- examples include Spanish, Hindi, Farsi, Mandarin and Arabic. Candidates who are also bi-cultural and who have lived experience as a survivor are preferred. ESSENTIAL FUNCTIONS: • Provide intensive crisis intervention and case management services to survivors who are eligible for housing assistance and subsidies from SAVE., • Provide housing assessments, housing problem solving services and other housing placement related activities in accordance with HUD guidelines and local Alameda County Gender Based Violence Survivor Coordinated Entry System (GBV/CE) guidelines., • Maintain all GBV/CE required documents in the database., • Attend required GBV/CE meetings, participate in related trainings and work to build relationships with partner agencies and providers., • Assist survivors to apply for and obtain benefits for which they are eligible (SSI/SSDI, CalWORKS, VOC). Provide necessary advocacy with governmental agencies., • Assist survivors in obtaining the documents required in order to demonstrate eligibility for housing opportunities., • Conduct individual case management sessions with survivors to identify needs, set goals, make referrals, and track survivor progress., • Assist survivors to identify and enroll in school and/or employment training programs that will enhance economic stability., • Provide ongoing case management, including: safety planning, risk assessment, and short-term and long-term intervention planning., • Identify obstacles and risks to the survivors' safety and stability, and work with survivors to overcome these obstacles and mitigate the risks., • Provide regular mobile or in-home advocacy and case management as appropriate., • Provide client transportation in agency vehicle and/or personal vehicle., • Provide follow-up/after-care case management services for survivors who are no longer receiving a subsidy., • Develop and/or conduct workshops on topics relevant to program participants (budgeting, time management, credit repair)., • Maintain a directory of resources that would benefit program participants., • Meet with the program manager for supervision regularly and as needed., • General Duties: (shared by all direct service staff), • Respond to crisis calls and walk-in survivors., • Track services provided and client information in the electronic database on a daily basis. Ensure electronic client files show current, updated information and changes in demographics., • Attend all required meetings, including staff meetings, trainings, consultations, and retreats., • Assist with training new staff members and volunteers., • Assume responsibility for site security and safety as needed., • Provide input into program planning and developments., • Assist with maintaining program operations and office functions as needed.
