Job Description Job Title: Medical Assistant / Community Health Worker (Hybrid) Department: Clinical & Public Health Services Reports To: Clinical Supervisor / Public Health Program Director FLSA Status: Non-Exempt Location: Omni Resource Services, Inc. – Public Health and Wellness Center Schedule: Full-Time / Part-Time Organizational Overview Omni Resource Services, Inc. is a Public Health and Wellness Center dedicated to advancing whole-person care by integrating clinical services, harm reduction, health education, and community engagement. Our mission is to address the root causes of health disparities through a coordinated approach to clinical care, outreach, and prevention. Position Summary The Medical Assistant / Community Health Worker (MA/CHW) plays a critical dual role in both clinical support and public health outreach. This position assists licensed providers and public health practitioners in delivering direct patient care and coordinating wraparound services that address social determinants of health (SDOH). The MA/CHW is a bridge between the clinical setting and the community—ensuring patients receive preventive services, chronic disease management, health education, and connection to resources that promote long-term health and wellness. Key Responsibilities 1. Clinical Support Functions • Greet and room patients, obtain vital signs, and document medical history and SDOH screenings., • Prepare exam rooms, assist with physical exams, immunizations, and point-of-care testing., • Conduct preventive screenings and support care gap closures (e.g., blood pressure, diabetes, STI/HIV/HCV testing, vaccinations)., • Support providers in coordination of chronic disease management and medication adherence., • Accurately document clinical encounters in the Electronic Health Record (EHR)., • Maintain infection control, safety protocols, and clinical supplies. 2. Community Health & Outreach • Engage patients in the clinic and in the community to promote health education and preventive services., • Conduct outreach for wellness events, vaccination drives, chronic disease screenings, and harm reduction activities., • Provide culturally competent education on topics such as infectious disease prevention, chronic illness, reproductive health, and substance use harm reduction., • Assist individuals with navigating Medicaid/Medicare benefits, insurance enrollment, and community resources (e.g., housing, food access, transportation)., • Follow up with patients in the community to support treatment adherence, appointment reminders, and care plan goals., • Participate in outreach efforts with CHWs, peers, and case management teams to reduce health disparities and increase access to care. 3. Care Coordination & Navigation • Schedule appointments and assist patients in accessing clinical, behavioral health, and supportive services., • Identify and address barriers to care, including transportation, coverage, or literacy gaps., • Facilitate referrals to internal and external programs, track outcomes, and provide follow-up., • Support population health initiatives through data collection and reporting., • Collaborate closely with interdisciplinary teams to support continuity of care. 4. Public Health Program Support • Support grant-funded initiatives related to preventive health, harm reduction, chronic disease, or behavioral health programs., • Assist with data entry, outreach logs, reporting, and performance measures for public health programs., • Participate in community health fairs, mobile clinic events, and wellness workshops., • Promote health equity and trauma-informed care in all interactions. Qualifications • High School Diploma or GED required., • Completion of an accredited Medical Assistant training program required., • Current MA certification or registration preferred., • Completion of, or willingness to complete, Kentucky Office of Community Health Workers certification or training., • 1–2 years of experience in a clinical or community health setting preferred., • Current CPR/BLS certification required., • Knowledge of public health principles, preventive care, harm reduction, and population health., • Experience with EHR systems, documentation, and care coordination., • Strong communication skills, cultural humility, and ability to work in community settings.
