The RN works independently or as part of a home healthcare team to deliver high-quality care while coordinating with physicians, caregivers, and interdisciplinary teams. \n\nKey Responsibilities:\n\n • Perform comprehensive patient assessments, including physical, emotional, and social needs, to develop individualized care plans.\n, • Administer prescribed medications, wound care, IV therapy, and other treatments as required.\n, • Educate patients and families on health conditions, medication management, self-care, and disease prevention strategies.\n, • Monitor patients for changes in health status, responding to acute issues such as infections, medication reactions, or chronic condition exacerbations.\n, • Collaborate with physicians, specialists, and interdisciplinary teams to coordinate care, ensuring continuity and improved health outcomes.\n, • Provide wound care, dressing changes, catheter care, and other specialized nursing interventions.\n, • Assist patients with mobility, self-care, and activities of daily living (ADLs) when necessary.\n, • Promote infection control and safety in the home setting to prevent complications.\n, • Document patient care, progress, and outcomes accurately and thoroughly, ensuring compliance with healthcare regulations.\n, • Advocate for patients by connecting them with community resources, support services, and medical professionals as needed.\n, • Provide emotional support and counseling to patients and families dealing with chronic illnesses, injuries, or end-of-life care.\n, • Educate caregivers and family members on how to care for patients, ensuring proper techniques and safety practices.\n\n\nQualifications:\n\n, • Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.\n, • Licensure:\n\n, • Valid Registered Nurse (RN) license in the state of practice.\n\n\n, • Certification:\n\n, • Basic Life Support (BLS) certification required.\n, • Home Health Nurse Certification (HHNC) preferred.\n\n\n
