The Clinical Manager, Home Care, is responsible for providing leadership and oversight of the Home Care clinical team members, including nurses and personal care aides. This role ensures high-quality, compliant, and person-centered, quality care while also serving as a clinical resource for the Goodwin Living At Home (GLAH) program. Key responsibilities include overseeing supervisory visits, reviewing and approving care plans, developing and managing medication management services, and conducting functional assessments for GLAH members. The Clinical Manager is also accountable for ensuring timely and accurate entry of required data into weekly and monthly Dashboard reports. In collaboration with clinical and operational teams, this role evaluates the efficiency and quality of home care services and ensures service excellence, regulatory compliance, and continuous improvement in Home Care delivery. Job Duties Supervision and Management of Nurse Supervisor(s): • Provide leadership and guidance to ensure effective team management and collaboration., • Conduct regular performance evaluations, offering constructive feedback, coaching, and professional development opportunities., • Deliver ongoing support and training to maintain the knowledge, skills, and resources needed to uphold high standards of care., • Support Supervisors in addressing staffing, performance, or compliance issues, providing direction and resources to resolve concerns effectively., • Oversee the scheduling and completion of supervisory visits for Home Care staff, ensuring compliance with regulatory requirements and organizational policies., • Monitor the quality and effectiveness of supervisory visits, ensuring staff receive appropriate guidance and that care consistently meets member needs and standards., • Review and approve care plans to confirm accuracy, completeness, and alignment with each member’s needs and preferences., • Ensure care plans are updated as conditions change, incorporating new assessments and team feedback as appropriate., • Oversee the medication management program, ensuring Home Care staff follow established protocols for medication reminders, set-ups, documentation, and member education., • Conduct periodic audits to confirm that medications are managed safely and that staff adhere to best practices in medication support., • Support Supervisors and staff in addressing member or family concerns related to medication management, collaborating with medical providers and pharmacists when clarification or coordination is needed., • Collaborate with the interdisciplinary team to integrate assessment results into care plans, recommending adjustments or interventions as appropriate., • Assist in assessing residents for Home Care Service, completing supervisory visits and supporting CNAs with care delivery and training as needed., • Ensure all clinical assessments and related documentation are completed accurately, on time, and in compliance with regulatory requirements and organizational standards., • Support staff with clinical questions and assist in troubleshooting issues related to patient care. Assist in coaching and mentoring home care assistant to be compassionate – motivated and have the heart to provide exceptional care and service to resident., • Lead the planning and execution of annual skills training for Home Care staff, ensuring competency validation, regulatory compliance, and alignment with best practices., • Conduct functional assessments for GLAH members as requested., • Assist in the monitoring of operational targets and ensure all required clinical data for the Home Care Dashboard is entered accurately and promptly., • Reviews/monitors operational variances with the data to ensure that key performance indicators (KPIs) are met and identify areas of concern that need attention. Foster open communication and collaboration among Home Care team members, including Supervisors, caregivers, and interdisciplinary healthcare providers. • Serve as a clinical resource by addressing questions and providing guidance on member care issues., • Promote a culture of accountability and exceptional service, directly supporting front-line aides in upholding high standards of care., • Partner with the Operations Manager to align with scheduling coordinators, reinforcing service excellence and ensuring consistency in the member and family experience, and provide back-up support when needed., • Partner with the scheduling team to help ensure resident and client satisfaction, coordinating effectively both business hours and after-hours on-call coverage., • Lead the Home Care Quality Assurance and Performance Improvement (QAPI) program, ensuring continuous monitoring, evaluation, and improvement of care and services., • Maintain ownership of the Home Care emergency preparedness list and ensure it is accurate, up to date, and accessible at all times., • Serve as the clinical lead during state surveys, demonstrating compliance, answering clinical inquiries, and ensuring required documentation is available., • Ensure the program remains survey-ready at all times by proactively monitoring compliance with state and federal regulations, accreditation standards, and organizational policies., • Participate in audits and quality initiatives, implementing corrective actions and best practices as needed to support service excellence., • Develop, communicate, and enforce any departmental/organizational policies and procedures., • Assist in managing the scheduling of Supervisors and Home Care staff to ensure adequate coverage for member needs across all shifts., • Collaborate with the Scheduling team to align staffing resources with care plans and member preferences, supporting continuity and satisfaction., • Provides clinical input when a shortage affects member safety or requires clinical judgment (e.g., whether a CNA vs. LPN is appropriate to cover certain tasks)., • Escalates concerns if staffing shortages compromise quality of care or compliance., • Participate in special projects, initiatives, home care strategy meetings, and committees assigned by the Executive Director of Home Care and GLAH., • Active Registered Nurse (RN) license in the state of practice., • Bachelor of Science in Nursing (BSN) preferred; equivalent experience may be considered in lieu of education., • Minimum of 5 years of experience in home care, long-term care or related healthcare settings, with at least 2 years in a supervisory or management role., • Strong experience in managing and supervising clinical staff., • In-depth knowledge of home care regulations, policies, and best practices., • Excellent leadership, management, and communication skills., • Strong clinical knowledge, with the ability to assess patient needs, create care plans, and evaluate care outcomes., • Familiarity with medication management processes and regulatory requirements., • Proficient in Microsoft Office Suite (Word, Excel, Outlook) and home care management software., • Ability to analyze data and generate reports for management., • Strong problem-solving skills and attention to detail.
