So, it is our pleasure to announce that we are currently seeking a full-time candidate for the role of Case Manager Supervisor (CMS) to join our team! – Veterans preferred for specific VA Shelter locations. By ICL definition, under the general supervision of the Dir. of Social Services, the Case Mgr. Supervisor is someone primarily responsible for providing day-to-day oversight and coordination to assigned case mgmt. staff. Occasionally, substituting for the Dir. of Social Services in their absence, assisting in the selection, training, and supervision of assigned staff. MIN. ROLE REQUIREMENTS & PREFERENCES: • LOCATION: New York, NY 10038 + Available Monday to Sunday, with options for O.T. – Shift: Days & Hours TBD, • EDU: Bachelor’s Degree – in a recognized mental health discipline (social work, psychology, nursing, rehab counseling, etc.), • Qualifications including CASAC-T or CASAC – preferred but not required, • EXP 1: 1+ years of shelter supervisory exp. – preferred but not required, • EXP 2: 3+ years of providing direct services to individuals with mental illness – preferably case mgmt. or outreach services ESSENTIAL KNOWLEDGE, SKILLS, AND ABILITIES: • Ability to stand, stoop, bend, and stretch for long periods of time, • Strong problem identification and solving skills with proposed solutions, • Ability to consistently report for work as scheduled on a consistent basis, • Ability to effectively use computer software such as MS Word and Outlook, • Ability to coordinate the activities of staff and integrate as part of a multidisciplinary team, • Ability to perform medium-to-heavy manual labor, including lifting objects weighing up to 25+ lbs, • Ability to work in a setting with diverse individuals with mental illness and/or behavioral disorders, • Ability to work independently and to conform to all applicable safety and accountability measures, • Ability to prepare accurate and timely documentation, reports, and other written material as assigned, • Strong communication, listening, interpersonal, writing, and conveying skills – bi-lingual is a plus but not mandatory, • Ability to learn, understand, and comply with all state and city regulations, policies, and procedures regarding case mgmt., • Ability to continually learn and apply knowledge with a willingness to participate in in-service training and development activities, • Ability to facilitate meetings and organize time effectively to accomplish all tasks in a timely way and meet deadlines without prompting, • Ability to follow all safety guidelines in accordance with the performance of the tasks mentioned below... CASE MANAGER SUPERVISOR TASKS: 1. STAFF COORDINATION & COMPLIANCE: • Assist in ensuring staff compliance, • Conduct staff supervision in accordance with program and division policy, • Educate assigned staff on the benefits and issues of working with residents and families, • Coordinate the activities of the staff in providing services to clients and facilitate resident groups, • Regularly inspect staff case records to evaluate the completeness and quality of treatment plans, • Ensure that the charts by staff are audit-ready and in compliance with all applicable agencies and funded regulations, • Monitor services performed by staff, either individually with a resident or in groups, to evaluate the quality of the service, • Schedule and ensure that assigned staff have attended both required in-service training courses and training provided by vendors, • Facilitate staff knowledge regarding types of clinical services, recreational activities, and ICL programs available, as well as community resources, • Ensure assigned staff follow the agency's time and attendance policy; recommend disciplinary action to the supervisor for those out of compliance, • Ensure staff are obtaining info, such as medical, psychological, and social factors contributing to the residents’ situation, and that the staff is evaluating the residents’ capabilities, • Ensure that staff are accompanying residents on regularly scheduled or emergency visits to medical treatment facilities, social agencies, government offices, or other locations associated with the treatment or assistance of the residents 2. CLINIC SAFETY & SERVICES: • Assign caseloads and related duties, • Review crisis assessments of assigned residents with the staff, • Ensure compliance with agency safety and infection control policies, • Assist in fostering the development of resident involvement committees, • Monitor the quality of the rehabilitative services provided to assigned residents/residents, • Develop, evaluate, implement, and modify a case management plan, meeting all deadlines and productivity standards, • Manage resources and multiple projects by delegating tasks and ensuring the completion of all tasks assigned and delegated, • Immediately report serious incidents, allegations of abuse, incidents, or sensitive situations, and complete incident reports to their supervisor, • Review intakes to ensure compliance and the timely processing of all required forms and processes, g., SSI clearance, HRAs, client documents, etc., • Review nonverbal and verbal crisis intervention techniques to be used with each individual resident/resident, and staff actions during a crisis situation, • Actively participates in recommending and implementing improvements in processes, procedures, policies, etc. for clinics, and ensures compliance with changes, • As assigned, conduct training on clinical subjects, charting, fire safety plans, fire drills, emergency plans and procedures, building security, and site-specific exposure control plans, • Monitor staff morale and the climate of the work environment to stop and/or eliminate sexual harassment, or any form of employee harassment/discrimination by taking and informing the DSS of the complaints, and cooperating in any resulting investigation 3. ADMINISTRATION & SUPERVISORY: • Assist in the orientation and training of supervisees, • May be a member of an agency or division committee(s), • Complete written forms and reports in an accurate and timely manner, • Assist in ensuring that the agency's mission is implemented and supported, • Assist in ensuring internal program compliance with federal, state, city, and agency regulatory requirements, • Assist with the mgmt. of employee accident reporting and initial submission of workers' compensation claims, • Assist the DSS with staff performance mgmt., including completion of performance reviews, counseling, or disciplinary memos, • In the Director of Social Services (DSS), absence or with the Program Director's permission, attend or direct meetings as assigned, • Secure the cooperation of and work effectively with others, actively looking for ways to help people by being compassionate and hopeful, • May be directed to coordinate with the Dept of Quality Assurance and the Corporate Compliance Dept for staff assistance in the preparation of an audit, • Work with residents, residents’ families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences, • May be asked to participate in plans of corrective action, incident reports, and program evaluation reports, and recommend the action needed for improvement, • With the Director's permission, establish and maintain work schedules in accordance with agency policy and assignments of staff. – may be responsible for managing the completion of time clock edits of assigned staff within the timeframes established by the HR Dept., • Communicate effectively with staff, residents/residents, families, and the public, serving as a role model by listening to what others are saying, being aware of their reactions, understanding why they react the way they do, and asking questions as appropriate with awareness of self and one’s impact on others 4. PROPERTY MAINTENANCE: • Recommend the purchase of supplies and equipment, • Recommend disciplinary action for staff whose negligence resulted in ICL property becoming lost, stolen, missing, or damaged, • May assist in maintaining accountability and serviceability of all ICL property assigned for use by assigned staff, to include vehicles, • Conduct regular inspections of resident/resident rooms or apartments to ensure property accountability and serviceability, establishing key control procedures, • Inspect and properly secure all property and equipment, conducting regular inspections of vehicles and property to ensure cleanliness, accountability, and serviceability, • Report lost, stolen, missing, or damaged property to the Program Director, and if directed, conduct initial investigations of property that is lost, stolen, missing, or damaged 5. FISCAL CONTROL & ACCOUNTABILITY: • Provide oversight of the resident funds program and possibly manage/distribute said funds, • Assist in the development of the internal money management census at expected occupancy levels, • Monitor census, entitlements, and collections to maximize revenue. Job Description PURPOSE OF POSITION Under the direction of the supervising Assistant Director of Nursing in Community Care, provides community-based professional nursing care to patients in group and private homes, as well as those without housing across New York City. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and the provision of care supportive to and restorative of life and well-being. Assesses, plans, implements, and evaluates the patient’s plan of care. Provides direct skilled nursing care and observes and instructs the patient in their place of residence. May coordinate services of other caregivers. AREAS OF RESPONSIBILITIES 1. Provides services requiring substantial and specialized nursing skill, in accordance with the plan of treatment signed by the physician and makes initial evaluation visits to clients with medical illnesses, chronic diseases, behavioral health disorders, etc. 2. Identifies and distinguishes between physical and psycho-social signs and symptoms. Selects and performs those therapeutic measures which are essential for effective execution of the nursing regimen for assigned patients/clients. 3. Develops, implements and continuously evaluates the nursing regimen for assigned patients/clients. May perform assessment of behavioral health needs, wound care, catheter insertion, medication teaching, diabetic care, patient/family education and counseling and aseptic technique. 4. Completes comprehensive patient assessments and develops, prepares and maintains individualized client care progress records, including required Outcome and Assessment Information Set (OASIS) documentation. 5. Records and maintains nursing care plans and progress notes on patients/clients to ensure continuity of care and initiates, develops and implements revisions as needed. 6. Interprets and reports responses of patients/clients to appropriate members of the health team and makes referrals to other disciplines in collaboration with physician, client and family, as appropriate. 7. Maintains continuity of client care by liaising with other health professionals assigned to the client including reporting signs, symptoms and changes to the patient’s condition to appropriate parties. 8. Provides teaching, guidance and direction to allied nursing personnel in rendering patient care., 9. Participates in departmental and interdisciplinary conferences pertaining to policies and procedures affecting nursing practice. 10. Interprets philosophy and objectives of agency and Nursing Department to patients/clients, families and other groups. 11. Maintains professional competence through participation in continuing education and other appropriate learning experiences, including agency provided in-service programs. 12. Participates in internal/external quality assurance/performance improvement (QA/PI) activities, programs, and training, as required. 13. Performs other professional nursing activities and related duties, as delegated by nursing service administration, including clinical record reviews. 14. Provides individualized skilled nursing care to the patients requiring part-time intermittent professional nursing service through the use of the nursing process. 15. Provides assessment of patient and develops a plan of treatment and care with ongoing evaluation of patient/family needs for other services. 16. Provides professional nursing services as outlined in the patient's plan of treatment and authorized by patient's physician, and provides emergency treatment as needed. 17. Records physical, social, psychosocial and environmental needs of the patient. Provide ongoing monitoring of the patient in the home care setting and escalates any clinical changes to the physician or non-physician provider as needed. 18. Utilizes input from patient, caregiver, MD and peers to devise a Nursing Care Plan which is feasible within the physical, financial and emotional resources of the patient/caregiver and the Department of Health regulations. 19. Evaluates patient's response to plan of care as necessary, consistent with physician's orders., 20. Reviews and revises plan of care as necessary, consistent with physician's orders., 21. Plans activities necessary for effectuating the timely discharge/transfer of patient, and notifies all appropriate parties including patient's physician. 22. Collaborates with case managers and other disciplines regarding the patient’s progress and discharge plans., 23. Demonstrates a professional attitude and ethical conduct in compliance with departmental standards., 24. Involves the patient and caregiver actively in goal setting., 25. Establishes and maintains a cooperative, effective working relationship with the staff, physician and nonphysician providers, Case Managers and other hospital personnel, outside vendors, agencies and the community at large. 26. Attends weekly to bi-monthly team meetings and participates in case conferences and case load reviews on a weekly to bi-weekly basis. 27. When needed completes Physician’s Orders in a timely manner., 28. Completes admission paperwork, including OASIS, Physician and Nursing POCs and progress notes within 48 hours of visit encounter. 29. Reviews clinical records to ensure compliance with departmental and regulatory standards., 30. Obtains written orders for all verbal orders returned., 31. Maintain up-to-date patient files with complete records including care plans and re-certifications, reports of all services rendered, progress notes and discharge summaries. 32. Writes clean and concise progress notes that reflect the implementation of the plan of care and the patient’s response to that plan of care. 33. Competent in knowledge of patient teaching plans and interventions., 34. Competent in knowledge of safe administration and teaching medications., 35. Demonstrates safety and accuracy in performing treatments and procedures according to the Home Care Department standards of care/practice. 36. Competent to complete thorough and accurate nursing assessments., 37. Demonstrate competence in skilled nursing interventions., 38. Competent in knowledge of documentation requirements for Home Care., 39. Participates in case conferences and/or Interdisciplinary Team conferences as needed and as designated by the Clinical Manager. 40. Works with other members of the Interdisciplinary Team to develop appropriate interventions in order to achieve clinical and functional goals. 41. Participates in the agency’s Quality Assurance Performance Improvement/PIP as assigned., 42. Supervises and monitors the care given by home health aides and ensures proper documentation., 43. Participates in an interdisciplinary health care team in determining and implementing a comprehensive plan of care. 44. Participates in the coordination and supervision of other professional and para-professional services in the home and provides supervision as needed and per DOH regulations. 45. Provides patient/family/significant other/patient identified representatives with teaching and involves them in assessment activities and goal setting. 46. Maintains current, complete, and appropriate documentation in the patient care record. May be required to use computer systems in carrying out these functions. 47. Duties include but may not be limited to wound care, including NPWT/VAC, Peg/Trach Care Management, Foley insertion and catheter care management, JP/PleuRx Drain care, administering injections. 48. Provides teaching on disease and medication management., 49. Obtains photographs for consultation by wound specialists., 50. Ability to apply evidence-based practices to the home care setting., 51.
