Job DescriptionJob Title: Intake Field Nurse (RN) Location: Brooklyn, NY Employment Type: Full-Time Language: Spanish, Russian, Mandarin or Cantonese preferred Position Overview Are you a Registered Nurse with strong assessment skills and bilingual capabilities? We’re partnering with a mission-driven healthcare organization seeking an Intake Nurse (RN) to support new members entering Managed Long-Term Care services in Brooklyn. The Intake Nurse will assess potential new members for program eligibility and safety, conduct UAS (Uniform Assessment System) assessments, explain available services, and develop an initial plan of care. This role involves working closely with members, families, physicians, referral sources, and clinical teams to ensure a smooth transition into managed long-term care. Responsibilities Conduct community outreach and meet with potential members in their homes and other community settings. Perform initial assessments using the UAS (Uniform Assessment System) tool. Evaluate potential members for program eligibility, safety, and care needs. Determine care needs and develop an initial plan of care. Explain the program and available services to members and their families. Communicate with physicians, referral sources, families, and clinical teams. Complete enrollment documentation accurately and within required time frames. Serve as a community resource for intake-related questions and needs. Consider members’ mental, social, cultural, and developmental needs when conducting assessments. Support a seamless transition for new members into managed long-term care. Qualifications Active New York State RN license required. Associate’s Degree in Nursing required; Bachelor’s Degree preferred. Minimum 1 year of acute care nursing experience required. Home care and/or MLTC intake experience preferred. Bilingual in Spanish, Russian, or Chinese required. Strong assessment, communication, critical thinking, and problem-solving skills. Knowledge of Medicare and Medicaid regulations preferred. Ability to work independently in a mobile/community-based environment. Important Details Primary Focus: UAS assessments, member eligibility, intake, outreach, and initial care planning Travel: Over 50% mobile; regular travel throughout the assigned territory Driver Requirement: Valid driver’s license, personal vehicle, and verifiable auto insurance required Territory Requirement: Must reside in the assigned service territory Schedule: Flexible hours; occasional evenings and/or weekends may be required Salary: $100,000–$135,000 per year, based on experience Benefits: Health, dental, vision, retirement plan, paid time off, professional development, and employee wellness programs Language Requirement: Spanish, Russian, or Chinese bilingual proficiency required Immediate Opportunity!
