More info at Akidolabs.com. The Opportunity Join our Street Medicine team as the vital link between street-based outreach and the healthcare system. This role splits between office-based administrative work (70%) and field clinical support (30%), focusing on care coordination and navigation for our most vulnerable patients through our Enhanced Care Management (ECM) program. What you'll do Administrative & Care Coordination (70%) • Manage referrals to specialists: initiate, schedule, track, and ensure follow-up, • Secure prior authorizations for medications, imaging, and specialist visits, • Verify insurance eligibility (Medi-Cal, Medicare) and assist with enrollment issues, • Maintain medical supply inventory for field outreach, • Document patient encounters, referrals, and authorizations in EHR system, • Support ECM program through patient tracking, scheduling, and care coordination, • Serve as office point of contact for patients, pharmacies, and community partners, • Accompany provider (MD/DO/NP/PA) on street outreach to encampments, parks, and shelters, • Build trusting relationships with patients and perform field intake, • Take vital signs, assist with clinical procedures, wound care, and specimen collection, • Prepare and administer medications/vaccinations under supervision, • Other duties as assigned Who you are Required: • Medical Assistant certification from accredited program, • Current BLS certification, • High School Diploma or equivalent, • Valid California Driver's License with clean record, • Strong computer and organizational skills, • Bilingual (English/Spanish), • 1+ years clinical administrative experience with prior authorizations/referrals, • Health benefits include medical, dental and vision, • Paid sick time in accordance in CA law., • Accrued paid time off (PTO) Physical Demands: Work may include both sedentary office duties and active engagement in the field, requiring walking and standing for extended periods.
