Job DescriptionAbout the Role: We're excited to offer an extremely unique opportunity for a Physician Assistant or Nurse Practitioner to work in a dynamic patient-centered practice that combines the healthcare models of MaxHealth and Elevate. An innovative, independently owned Family, Internal & Sports Medicine clinic 25 years strong is looking for a provider who is excited to help lead a collaborative team. MaxHealth-Elevate has earned a reputation as being one of the most innovative and integrated practices across the state. In addition to traditional primary care, MaxHealth-Elevate offers services in exercise and nutritional counseling, injury recovery and prevention, brain injury and mental health to name a few. We pride ourselves on managing schedules so that patients are seen on time and focus on schedules and support to achieve work/life balance. In addition to a wide range of patient support service unlike what you have seen in any other primary care setting, we also are breaking the mold on how care is delivered. Our consumer forward, tech enabled clinics are working to shed the traditional fee-for-service care delivery models so patients can receive care the way they prefer, and providers can deliver care in ways that make sense. FAMILY MEDICINE SPECIFIC POSITION REQUIREMENTS: 1. Systematically assesses patients' health status, arrives at appropriate diagnosis(es) and implements care plan for effective clinical outcomes., 2. Contributes to effective and efficient practice management policies and practices that support patient access to medical care., 3. Manages patient care through appropriate use of available resources., 4. Provides leadership for Practice and community relations., 5. Demonstrates commitment to fellow employees., 6. Demonstrates commitment to the Practice. WHAT WE'RE LOOKING FOR: • Must hold a current valid medical license in the State of Texas (PA or FNP), • Licensed Physician Assistant in Texas, • Licensed Family Nurse Practitioner (FNP) in Texas are encouraged to apply, • Primary care or family medicine experience required, • Complies with all relevant laws, regulations and policies, • Contributes to a safe working environment, • Compassionate, patient-centered approach, • Demonstrated leadership, teaching, evaluation and communication skills NOTE: This job description is not intended to be all-inclusive.
