JOB SUMMARY/OBJECTIVE: The coordinator works in collaboration with the Drug Treatment Court (DTC) team and in conjunction with Tuerk House, Inc. to develop and monitor plans of care for an assigned caseload of DTC participants (“clients”). This position coordinates services and clinical care identified as a result of a comprehensive assessment provided by a BHSB-contracted assessor. The coordinator ensures their assigned clients’ changing needs are met with appropriate type(s) and intensity of service(s). This includes regular meetings with each client, treatment providers, and members of the DTC team to assess the effectiveness of the care plan, assess the client’s adherence to the plan, and revise the plan as needed. This position reviews and checks the quality of treatment providers’ ongoing monthly documentation of treatment progress and clients’, progress notes from provider, and Agents Care note, as well as participant continuing care plans. The coordinator investigates and troubleshoots complaints from offenders as well as criminal justice stakeholders. This position acts as a liaison between BHSB, criminal justice entities, and service providers. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Leads in the development, implementation, and provision of care coordination services for DTC participants., • Cultivates positive, pro-active communication with other system partners related to care coordination needs., • Maintains detailed knowledge of the levels of care in the public behavioral health system, providers qualified to provide each level of care, and intake processes for providers., • Creates detailed documentation of day-to-day work, including but not limited to treatment placement records, care plans and care plan updates, client progress reports, client discharge plans, correspondence with criminal justice stakeholders and service providers, and other documentation as appropriate., • Facilitates transportation of clients to appointments as necessary., • Collaborates with treatment and service providers in designing care plans and treatment plans with clients. Provides technical assistance to treatment and service providers related to the public behavioral health system and DTC policies & processes., • Supports the DTC in following evidence-based practices related to behavioral health in criminal justice settings., • Cultivates collaborative, mutually supportive relationships with DTC Peer Recovery Specialists and DTC Case Managers, Probation Agents, Public Defender, State’s Attorney, and the DTC Judge., • Performs other duties as assigned which are consistent with the general responsibilities of the position. Quarterly BHSB Reports., • Work effectively and collaboratively with others, both internally and externally., • Serve as part of BHSB’s disaster response team as needed, in any capacity that is needed., • Perform other duties as assigned. REQUIRED EDUCATION, CERTIFICATIONS AND/OR EXPERIENCE: • Bachelor's degree in social work, Addiction, Human Service, Psychology or related field required/ master's degree preferred, • Three (3) years of professional experience in behavioral health services., • Valid Maryland Driver’s License, • Ability to travel locally 50% of the time is required, • Required to travel to other locations for court, site visits, meetings and other business-related needs, • Required: an unrestricted, active certification or license by the Maryland Board of Professional Counselors or other specified Health Occupations Act., • Licensed Clinical Professional Counselor (LCPC), • Certified Professional Counselor – Alcohol and Drug (CPC-AD), • Certified Associate Counselor – Alcohol and Drug (CAC-AD), • Licensed Clinical Social Worker (LCSW), • Licensed Clinical Alcohol & Drug Counselor (LCADC), • Professional experience working with the criminal justice system preferred. REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: • Extensive knowledge of mental health and substance use resources and treatment issues., • Extensive knowledge of the criminal justice system., • Ability to work effectively with community organizations, community leaders, service providers, judges, lawyers, and consumers., • Able to make productive contributions through talent, knowledge, skills, and good work habits., • Proactively initiates plans and convenes stakeholders to improve systems-level work, • Extensive knowledge of community resources., • Independently takes new tasks and responsibilities in a proactive way, • Experience working within a multidisciplinary team, • Experience with determining when issues should be escalated to a higher level, • Personal belief in recovery and the recovery process., • Excellent written and verbal communication skills, with the ability to interact effectively with staff, clients, and the public., • High energy, flexibility, and strong interpersonal skills to work effectively within a team., • Ability to engage clients flexibly based on their individual needs., • Respect for and maintenance of professional boundaries with all individuals., • Ability to follow written and verbal instructions accurately., • Proficiency in reading, understanding, and speaking English. Background: This position requires successful background and drug test completion Physical Requirements: • Prolonged periods of sitting at a desk and working on a computer., • Must be able to lift up to 15 pounds at times., • Prolonged periods of standing and bending.
