Job DescriptionDescription: The ACT Co-Occurring Specialist provides skills training, case management, and crisis intervention to participants with co-occurring disorders within the multidisciplinary assertive community treatment (ACT) team. LHC mission and values stipulate that all services provided are based on participant strengths and choices and follow evidence-based practices of psychosocial rehabilitation. Essential Job Functions: • Conduct all phases of psychosocial rehabilitation and outreach (engagement, assessing strengths, goal planning, safety planning, evaluating and summarizing progress), • Complete intake and annual substance use assessments with participants in a timely fashion, • Provide and coordinate substance use treatment, including individualized services tailored to participant needs and their stage of change readiness, • Create and review relapse prevention plans, wellness management plans, and safety plans with participants, • Assist with developing/utilizing natural supports and accessing community resources that support recovery, • Facilitate groups (e.g., EIMR, DDA) for individuals with co-occurring disorders, • Provide independent living skills training (e.g., medication support, transportation, symptom management, household management), • Provide case management and crisis intervention services to participants, • Collaborate with other staff members, community agencies/supports, and family members about participants mutually served, • Complete required documentation within the identified time standards, including daily progress notes, • Actively support participant self-reliance using outreach, motivational interviewing, and cognitive- behavioral techniques, • Participate in daily multidisciplinary team meetings, • Participate in the on-call rotation for the ACT team crisis line, • Provide back-up services for peer team members as needed, • Provide cross-training to other ACT staff on the team to help them develop co-occurring disorder assessment and treatment skills, • Knowledge and skills related to the treatment of mental health disorders, • Knowledge of and/or experience with trauma-informed services and systems of care, • Ability to exercise discretion and independent judgment in all areas of job performance including adherence to appropriate relationship boundaries and HIPAA confidentiality, • Ability to communicate appropriately with staff and clients, including individuals with SMI symptoms, • Ability to apply clinical judgment, ethics, and accountability to formulate and implement treatment plans and other clinical documentation, • Understanding of and adherence to the ethical standards of the governing license or certification board, • Ability to provide supportive, professional, and appropriate services to individuals, • Ability to effectively engage clients by telephone, audio/visual telehealth, and in person., • Ability to use mobile technology, computers, and standard office equipment, • Ability to enter clinical documentation into an electronic health record system., • Ability to complete a variety of tasks daily (e.g. phone calls, outreach, referrals, EHR documentation, etc.), • Ability to be a resource for others and work with providers in the community, as well as knowledge of community resources., • Ability to offer understanding and respond effectively and with sensitivity to special populations, such as those defined by race, ethnicity, language, age, sex, sexual orientation, economic standing, disability, migrant and seasonal worker status, homelessness, people living with HIV/AIDS, and others., • Ability to meet Universal Expectations of Employment, • Ability to remain in compliance with policies, procedures, regulations, and standards, • Ability to perform physical requirements of the position, including lifting up to 20 pounds, standing, sitting, navigating stairs, bending, kneeling, crouching, reaching, grasping, using fingers, tolerating strong odors, speaking, repetitive motion, extensive use of computer, local travel.Requirements:, • Bachelor’s degree (B.S. or B.A.) in a social service discipline, • Must have CADC certification through MHACBO & ability to get QMHA certification at time of hire, • Minimum of one year of demonstrated experience serving individuals with co-occurring mental health and substance use disorders, • Ability to obtain a Medicaid Provider Number, • Valid Oregon Driver License in accordance with Laurel Hill Center vehicle use policy and procedures, • Documentation of successful completion of a background and fingerprint check as specified by Oregon law, as well as a urinalysis drug screen (Although recreational marijuana is legal in Oregon, our policy—aligned with both federal and state regulations—requires all applicants to test?negative for THC?prior to employment)
