Job DescriptionPosition Summary: Under the guidance of a Senior Supervisor, this position provides direct supervision, training and consultation to a team of 35 to 40 Home Care Aides. The Supervisor ensures that day-to-day service delivery is coordinated and provided efficiently and in accordance with best practice standards and the Illinois Department on Aging (IDOA), Community Care Program, In-Home Services policies and procedures. Essential Duties and Responsibilities Include the following, other duties as assigned: Program Operations Effectively utilizes the program’s time & labor software and other systems used for program operations. Maintains and utilizes an effective tracking system to continuously monitor direct service staff assignments, service delivery and participant information (e.g. authorized service units and high risk participants) and personnel-related processes (e.g. quarterly home visitation, supervision, performance evaluation schedules). Manages and monitors assigned staff’s work hours and collects all service calendars in a timely manner. Reviews all information for accuracy before submission to the Billing Specialist. Completes and submits a monthly program report according to established deadline (5th of each month). Reviews information for accuracy and comprehensiveness before submission. Ensures that all HCA personnel and participant files are well organized, accurate and up-to-date, according to IDOA requirements. Documents all introductory and ongoing supervisory visits, as well as monthly wellbeing phone calls and other contacts and service activities in a timely manner. Prepares and reviews participants’ Plans of Care (as directed by IDOA/CCU or MCO units) and instructs the assigned HCA regarding their assigned schedule and the services they are to provide. Coordinates all related service schedules and implements authorized participant care plans. Conducts introductory home visits for all newly assigned participants to assess the participant’s home environment, introduce the assigned Home Care Aide and to provide key program and service delivery information. During the introductory home visit, the Supervisor also completes a comprehensive psychosocial and safety assessment. Conducts monthly wellbeing phone calls to all assigned participants, as well as quarterly home visitations to assigned participants. Conducts additional home visitation when staff identify concerns about the safety or wellbeing of participants, during worker reassignments or when participants request a Supervisory meeting. Monitors established plans of care and appropriate provision of services, as well as staff performance through home visitation and observation and monthly phone calls to participants. Provides participants with additional information and referral/linkage to other community services, when needed. May also assist participants in coordinating their annual redetermination-of-need assessments, via the IDOA/CCU or MCO care coordination units. Performance & Quality Management Maintains and demonstrates thorough knowledge of the Illinois Department on Aging (IDOA), Community Care Program, In-Home Services policies and procedures to ensure the program is in full compliance with requirements. Understands and follows Casa Central’s personnel and agency policies and is able to interpret them to assigned staff. Complete required documentation during shift and follow on-call notification procedures., • Provide safe transportation for youth or young adults and their support systems, following agency policy and state laws., • Follow guidelines and procedures to ensure accurate and safe distribution and proper documentation of medications., • Employ all aspects of de-escalation and behavior management techniques as outlined in CPI protocol. Restrain youth or young adults as necessary., • Actively engage in learning and integrating developmentally appropriate, attachment-informed, and trauma-responsive theory and practice consistent with the Mercy Model of Care. What we need from you: • At least 21 years of age., • Bachelor’s degree (preferred) or a High School diploma with at least 5 years of experience in youth development., • Demonstrated experience working with youth, ideally those who have experienced developmental adversity and/or trauma (at least 2 years’ experience preferred)., • Possess a valid Illinois driver’s license with the ability to safely drive a van, maintain a clear driving record, and obtain/retain driving approval through the Department of Children and Family Services., • Regular and predictable attendance., • Ability to have a flexible work schedule. We are currently looking for full-time – Data and Evaluation Administrator Starting at $50,000 – $55,000 contingent upon experience, education, etc. POSITION SUMMARY: The Data and Evaluation Administrator is passionate about utilizing data to support criminal justice reforms and ensure quality services are provided to clients involved in the criminal legal system. This position plays a key role in supporting TASC’s implementation of Pre-Trial Success Act (PSA) services, called Connections: Voluntary Community Support Program (C: VCSP). C: VCSP assists clients within the pretrial services system who have identified needs for behavioral health and recovery support services, including comprehensive screening and assessments for substance use disorders, connecting clients to treatment programs, monitoring their progress, and providing critical support services. The Data and Evaluation Administrator is responsible for overseeing data entry and quality, identifying data gaps and inconsistencies, and supporting data needs and requests both internally and externally. This individual will work collaboratively with cross-functional teams and respond swiftly to requests for data reports, visualizations, and presentations. This position plays a key role in data and implementation studies. The Data and Evaluation Administrator will work closely with TASC staff—operations, development, quality, compliance, and outreach—and external stakeholders to ensure quality documentation and proactively identify areas of improvement. The Data and Evaluation Administrator reports to the Director of Performance and Quality Improvement. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES: • Collaborate with system partners starting in Cook and Will Counties and continuing in jurisdictions throughout Illinois to integrate court clerk and other tracking systems and/or their data with TASC’s CMR; create bridges between siloed data sources and systems., • Develop and implement systems of interoperability between TASC’s CMR, Twilio, and other platforms as needed., • Work with the implementation team on the customization of new CRM software to ensure adequate mapping and data collection for pre-trial programming., • Provide ongoing feedback on potential improvements in CRM architecture and management to ensure appropriate data collection and analysis., • Manage/oversee all aspects of record-keeping for clients, including collection, digitization, and filing client records., • Supervise C: VCSP Data Coordinators., • Assist with initial and ongoing training of staff in data entry and continual quality improvements., • Work with project/program leadership to ensure data gaps and inconsistencies are reconciled on a regular basis, • Assist in the optimization of operational processes and systems to ensure efficiency and high-quality delivery of the C: VCSP model data infrastructure in collaboration with funders, PQI, and Operations., • Support PQI, directors, and finance in the management of funding and account expenditures as they relate to program administration, • Support TASC leadership and program partners in developing and presenting regular updates on program outcomes and utilizing data to inform best practices, mitigation strategies, and client support/advocacy., • Serve as the lead for Data Coordinators in managing administrative tasks, including tracking inventory and ordering & receipt of new supplies., • Generate alternative solutions to address client needs and work challenges., • Crosstrain and, when needed, fill in for other project roles., • Attend all trainings as required for agency and professional development., • Monitor the quality and timeliness of client data entered into CRM and review regularly to meet performance goals and improve program quality for clients., • Review C: VCSP files, creating reports and corrective action requirements for each file., • Create and present a summary report at monthly Audit Subcommittee meetings., • Work with program staff on corrective actions., • Conduct regular audits of data and reports to ensure quality and accuracy, • Develop C: VCSP Key Performance Indicators (KPIs) in collaboration with the Director of PQI; calculate and summarize KPIs monthly; identify and suggest data-informed areas for growth and project development., • Ensure that metrics on client outcomes and staff efforts are collected and reported to TASC leadership and other program stakeholders on a regular basis or as requested., • Work with the Director of PQI and other agency staff to design performance tracking and reporting tools., • Reviews discrepancies in data entered, requests clarification or advises supervisor of issues related to challenge in accordance with agency procedures. EDUCATION AND EXPERIENCE: • Bachelor’s degree in public policy, public health, sociology, social work, psychology, criminal justice, or a related field, etc. from an accredited college/university, Master’s degree preferred, • 3-5 years of experience using data to evaluate and demonstrate a need for new programs., • 1-3 years of experience supervising staff., • Familiarity with Cook County bond court, pretrial legal procedures, Cook County’s CCC Portal, and criminal justice reform landscape in Illinois, Cook County, Lake County, and Chicago., • Experience in large scale data management, project organization, data quality assurance, and task management., • Experience working with Twilio or Textedly in a business capacity preferred., • Deep understanding of how to structure data collection, storage, and analysis. Experienced in G suite and Microsoft Excel. Background in quantitative analytics a plus., • Commitment to racial justice and building an infrastructure to support lasting policy change.
