Basis: Fixed Term for 4 to 6 months Location: All Centres serviced by the Practices, currently including: Newbiggin Health Centre, Seaton Hirst Primary Care Centre. In the course of your role you will be required to visit other locations including patients homes. Clinical Accountability: The General Practitioners in Partnership Managerial Accountability: Practice Manager JOB PURPOSE: To provide personal medical care to patients of the Practice, working as part of the Practice team. • Provide high quality medical care to patients registered with the Practice., • Provide continuity of care where possible., • Work collaboratively with all members of the PHCT., • Comply with primary care clinical governance standards., • Actively contribute to achievement of highest possible QOF score, Enhanced Servicesand ICB requirements. Personal development is actively encouraged and supported. • Working in a large, busy primary care team across two sites, • Supporting provision of a safe primary care service, • Opportunities for development and training, particularly developing a special interest, • Working in a managed environment, • Supporting the continued achievement of high quality multi-disciplinary care, • Clinical competencies in full spectrum of Primary Care, • Ability to work as an integrated team member, • Excellent communication skills, • Working knowledge of SystmOne, • Certificate of Completion of Training from the Postgraduate Medical, Education and Training Board, • GMC Registration, • DBS Enhanced Disclosure, • Occupational Health Check, • Medical Indemnity Insurance, • On Performers List, • In accordance with the practice timetable, as agreed, undertake a variety of duties including surgery consultations and emergencies, telephone consultations and queries, triaging, visiting patients at home, checking and signing both acute and repeat prescriptions, dealing with queries and patient test results., • Making professional, autonomous decisions in relation to presenting problems, whether self referred or referred from other health care workers within the organisation., • Assessing the health care needs of patients with undifferentiated and undiagnosed problems., • Screening patients for disease risk factors and early signs of illness., • In consultation with patients and in line with current Practice disease management protocols, developing care plans for health., • Providing counselling and health education., • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate., • Timely assessment of incoming electronic and paper correspondence relating to both patient and non patient information as required, using the appropriate electronic systems., • To record clear and contemporaneous computer based consultation notes to agreed standards using SystmOne., • Prescribing in accordance with locally agreed or national guidelines., • Participate on a fair share basis in the Co-ordinator and Urgent GP corporate sessions., • Participate in training plans of GP registrar, medical and nursing students as required., • In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care., • Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety, annual QOF Assessment, Enhanced Services and ICB requirements., • A commitment to personal development and participation in audit and the Practices In-House Training Education & Development (TED) programme., • Participation in an annual individual performance review, annual GP Appraisal including taking responsibility for maintaining a record of own personal and/or professional development., • Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work., • Participation in regular mentor sessions with a designated Partner.
